【摘要】：目的探討男性T2DM患者性腺功能減退的原因。方法選擇男性T2DM患者156例,分為低睪酮(TT)組(n=59)和正常TT組(n=97),測定血壓、血糖、血脂、胰島β細(xì)胞功能,以及性激素[包括血清TT、性激素結(jié)合球蛋白(SHBG)、孕激素(P)、催乳素(PRL)、黃體生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)、脫氫表雄酮(DHEA)],比較兩組性激素、糖脂代謝指標(biāo)及胰島素等的差異,并分別按BMI及LH進行分組,比較各組間TT水平。結(jié)果與正常TT組比較,低TT組BMI、FIns、2 hIns及胰島素抵抗指數(shù)(HOMA-IR)升高(￡=3.76,2.03,2.68,2.13,P0.05),低TT組LH、FSH、SHBG、E2降低(t=-2.94,-2.16,-3.08,-3.50,P0.05);與高體重組比較,低體重組TT增加(P0.05);與低LH組相比,高LH組TT增加(P0.05)。相關(guān)分析顯示,TT與BMI、FIns及HOMA-IR呈負(fù)相關(guān)(r=-0.33,-0.18,-0.17,P0.05),與LH、FSH、E2呈正相關(guān)(r=0.14,0.16,0.28,P0.05)。結(jié)論男性T2DM患者往往會合并低促性激素性性腺功能減退癥。
[Abstract]:Objective to investigate the causes of gonadal dysfunction in male patients with T2DM. Methods 156 male patients with T2DM were divided into low testosterone (TTT) group (n = 59) and normal TT group (n = 97). Blood pressure, blood glucose, blood lipids, islet 尾 cell function and sex hormones were measured. [These include serum TTT, sex hormone binding globulin (SHBG), progesterone (P), prolactin (PRL), luteinizing hormone (LH), follicle stimulating hormone (FSH), estradiol (E2). The difference of sex hormone, glucose and lipid metabolism index and insulin were compared between the two groups, and were grouped according to BMI and LH. Results compared with the normal TT group, the BMIFIns2 hIns and the insulin resistance index (HOMA-IRR) of the low TT group were increased (3. 76). In low TT group, the level of estradiol decreased from 2.94 to 2.16 and 3.08 to 3.50 in low TT group. P0.05; Compared with the high body weight group, the TT of the low weight group increased P 0.05; Compared with low LH group, TT increased in high LH group (P 0.05). Correlation analysis showed that TT was negatively correlated with BMIFIns and HOMA-IR. (-0.17) P0.05A was positively correlated with FSHE _ 2 of LHH _ (1) 0.14 ~ (0.16) ~ (0.28). Conclusion male patients with type 2 DM are often associated with hypogonadal hypogonadism.
