后腹腔鏡保留腎單位手術(shù)治療老年T1a期腎癌的療效

【摘要】：目的分析老年T1a期腎癌患者進行后腹腔鏡保留腎單位腎部分切除術(shù)(LNSS)的有效性和安全性。方法老年T1a期腎癌患者103例依據(jù)手術(shù)方法不同分為兩組:LNSS組48例,平均年齡(65.1±3.0)歲;后腹腔鏡根治性腎切除術(shù)(LRN)組55例,平均年齡(64.5±3.1)歲;比較兩組手術(shù)時間、術(shù)中出血量和住院時間,并在術(shù)前、術(shù)后1 d、術(shù)后6、12個月取血測量血清肌酐。隨訪12~78個月,獲得患者的遠期療效。結(jié)果LNSS組手術(shù)時間、術(shù)中出血量和住院時間均顯著大于LRN組(P0.01);兩組圍術(shù)期并發(fā)癥(肺炎、腦梗死、肺栓塞)比率差異無統(tǒng)計學(xué)意義(P0.05);兩組術(shù)前、術(shù)后1 d以及術(shù)后6個月和12個月的血清肌酐水平比較差異均無統(tǒng)計學(xué)意義(均P0.05);兩組遠期療效比較差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論 LNSS保留了患者腎功能,臨床上對老齡局限性小腎癌患者安全有效。
[Abstract]:Objective to analyze the efficacy and safety of retrolaparoscopic partial renal unit renal resection (LNSS) in elderly patients with stage T1a renal carcinoma. Methods according to the different surgical methods, 103 elderly patients with stage T1a renal cell carcinoma were divided into two groups: LNSS group (n = 48, mean age (65.1 鹵3.0) years), retrolaparoscopic radical nephroresection group (n = 55, mean age (64.5 鹵3.1) years), operation time, intraoperative blood loss and hospitalization time, and serum creatinine was measured before operation, 1 day after operation and 6 months after operation. The patients were followed up for 12 to 78 months to obtain the long-term curative effect. Results the operation time, intraoperative blood loss and hospitalization time in LNSS group were significantly higher than those in LRN group (P 0.01). There was no significant difference in perioperative complications (pneumonia, cerebral infarction, pulmonary embolism) between the two groups (P 0.05). There was no significant difference in serum creatinine level between the two groups before operation, 1 day after operation and 6 months and 12 months after operation (both P 0.05). There was no significant difference in long-term curative effect between the two groups (P 0.05). Conclusion LNSS preserves the renal function of the patients and is safe and effective in the elderly patients with localized small renal cell carcinoma.
