非肌層浸潤(rùn)性膀胱癌術(shù)后復(fù)發(fā)的相關(guān)因素分析

【摘要】：目的：探討影響非肌層浸潤(rùn)性膀胱癌患者行經(jīng)尿道膀胱腫瘤電切術(shù)后復(fù)發(fā)的相關(guān)因素。 方法：回顧性分析2009年4月至2013年10月在福建醫(yī)科大學(xué)附屬第一醫(yī)院泌尿外科手術(shù)治療的159例非肌層浸潤(rùn)性膀胱癌患者的臨床和病理資料，選擇9項(xiàng)可能影響術(shù)后復(fù)發(fā)的指標(biāo)，用SPSS19.0軟件進(jìn)行單因素和多因素分析，，篩選對(duì)非肌層浸潤(rùn)性膀胱術(shù)后復(fù)發(fā)有影響的臨床病理因素。 結(jié)果：到隨訪期止，159例患者中29例術(shù)后復(fù)發(fā)，術(shù)后總復(fù)發(fā)率為18.24%，平均無(wú)復(fù)發(fā)生存時(shí)間為21.25個(gè)月。單因素分析表明影響非肌層浸潤(rùn)性膀胱術(shù)后復(fù)發(fā)的因素為腫瘤大小、組織學(xué)分級(jí)、既往病史、腫瘤分期、腫瘤數(shù)目（P＜0.05），多因素COX分析提示腫瘤分期（HR=2.741）、腫瘤數(shù)目(HR=2.293)是影響術(shù)后復(fù)發(fā)的獨(dú)立因素。 結(jié)論：腫瘤大小、組織學(xué)分級(jí)、既往病史、腫瘤分期、腫瘤數(shù)目是非肌層浸潤(rùn)性膀胱癌術(shù)后復(fù)發(fā)的相關(guān)因素。其中腫瘤分期、腫瘤數(shù)目是復(fù)發(fā)的獨(dú)立影響因素。
[Abstract]:Objective: to investigate the related factors of recurrence after transurethral resection of bladder tumor in patients with non-myenteric invasive bladder cancer. Methods: the clinical and pathological data of 159 patients with non-myenteric invasive bladder cancer treated by urology in the first affiliated Hospital of Fujian Medical University from April 2009 to October 2013 were analyzed retrospectively. nine indexes that might affect postoperative recurrence were selected. Univariate and multivariate analysis was carried out by SPSS19.0 software to screen the clinicopathological factors affecting the postoperative recurrence of non-myenteric invasive bladder. Results: at the end of the follow-up period, 29 of 159 patients had recurrence after operation, the total recurrence rate was 18.24%, and the average survival time without recurrence was 21.25 months. Univariate analysis showed that the factors affecting the recurrence of non-myenteric invasive bladder were tumor size, histologic grade, previous history, tumor stage and tumor number (P < 0.05). Multivariate COX analysis showed tumor staging (HR=2.741). Tumor number (HR=2.293) is an independent factor affecting postoperative recurrence. Conclusion: tumor size, histologic grade, previous history, tumor stage and tumor number are related factors for postoperative recurrence of non-myenteric invasive bladder cancer. Among them, tumor stage and tumor number are independent influencing factors of recurrence.
