980nm半導(dǎo)體紅激光前列腺汽化術(shù)與經(jīng)尿道前列腺電切術(shù)治療良性前列腺增生的臨床療效比較

【摘要】：目的本研究主要是總結(jié)980nm半導(dǎo)體紅激光前列腺汽化術(shù)在治療良性前列腺增生（Benign prostatic hyperplasia, BPH)方面的臨床療效及手術(shù)情況，并與經(jīng)尿道電切術(shù)（Transurethral prostatic resection, TURP）進(jìn)行綜合分析比較，探討980nm半導(dǎo)體紅激光前列腺汽化術(shù)治療BPH的安全性及療效，為今后980nm半導(dǎo)體紅激光在BPH方面的臨床應(yīng)用提供科學(xué)依據(jù)。 方法采集自2012年4月至2013年4月期間在河北聯(lián)合大學(xué)附屬醫(yī)院泌尿外科住院并接受980nm半導(dǎo)體紅激光治療或TURP治療的BPH患者，，隨機(jī)分成兩組，其中87例接受980nm半導(dǎo)體紅激光前列腺汽化術(shù)，81例接受TURP治療。觀察記錄兩組患者的手術(shù)時(shí)間、術(shù)中出血量、術(shù)后并發(fā)癥，術(shù)前及術(shù)后分別進(jìn)行國(guó)際前列腺癥狀評(píng)分(IPSS)、生活質(zhì)量指數(shù)評(píng)分（QOL）、最大尿流率（Qmax）、殘余尿量（PVR）等指標(biāo)的差異有無(wú)統(tǒng)計(jì)學(xué)意義，并進(jìn)行綜合分析比較。 結(jié)果兩組患者的年齡、病程、前列腺體積、PVR、Qmax、IPSS、QOL、血鈉值、血紅蛋白值等術(shù)前情況進(jìn)行比較，差異無(wú)統(tǒng)計(jì)學(xué)意義（P＞0.05）；980nm激光組手術(shù)時(shí)間大于TURP組，差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）；980nm激光組術(shù)中失血量、術(shù)后膀胱沖洗時(shí)間、留置尿管時(shí)間以及術(shù)后住院時(shí)間均明顯少于TURP組，差異有統(tǒng)計(jì)學(xué)意義（P＜0.01）；980nm激光組術(shù)后繼發(fā)出血、TURS綜合癥少于TURP組，差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）；與術(shù)前癥狀相比較，兩組患者術(shù)后較術(shù)前有顯著差異，差異有統(tǒng)計(jì)學(xué)意義（P＜0.01）；兩組患者術(shù)后療效比較，差異無(wú)統(tǒng)計(jì)學(xué)意義（P＞0.05），說(shuō)明兩組患者術(shù)后療效相同。 結(jié)論980nm半導(dǎo)體紅激光前列腺汽化術(shù)能明顯改善患者的下尿路梗阻癥狀，具有與TURP相同的近期療效，與TURP相比，具有手術(shù)操作簡(jiǎn)單、術(shù)中出血少、術(shù)后留置尿管時(shí)間及住院時(shí)間短、術(shù)后恢復(fù)快等優(yōu)點(diǎn)，是一種治療BPH安全有效的微創(chuàng)手術(shù)方式。
[Abstract]:Objective the purpose of this study was to summarize the clinical efficacy and surgical results of 980nm semiconductor red laser vaporization of prostate in the treatment of benign prostatic hyperplasia (Benign prostatic hyperplasia, BPH) and to compare it with transurethral resection of the prostate (Transurethral prostatic resection, TURP). To investigate the safety and efficacy of 980nm semiconductor red laser vaporization of prostate in the treatment of BPH, and to provide scientific basis for the clinical application of 980nm semiconductor red laser in BPH in the future. Methods from April 2012 to April 2013, BPH patients who were hospitalized in urological department of affiliated Hospital of Hebei Union University and received 980nm semiconductor red laser therapy or TURP treatment were randomly divided into two groups. Among them 87 cases were treated with 980nm semiconductor red laser vaporization of prostate and 81 cases were treated with TURP. The time of operation, the amount of bleeding during operation, the postoperative complications, the (IPSS), quality of life index (QOL),) and the (Qmax), (International Prostate symptom score) before and after operation were observed and recorded in the two groups. The difference of residual urine volume (PVR) and other indexes was statistically significant, and comprehensive analysis and comparison were carried out. Results there was no significant difference between the two groups in age, course of disease, prostate volume, serum sodium level of PVR,Qmax,IPSS,QOL, and hemoglobin value before operation (P > 0. 05), but no significant difference was found between the two groups (P > 0. 05). The operation time of 980nm laser group was longer than that of TURP group (P < 0 05). The blood loss, bladder irrigation time, indwelling catheter time and postoperative hospitalization time in 980nm laser group were significantly lower than those in TURP group (P < 0 01). TURS syndrome in 980nm laser group was significantly lower than that in TURP group (P < 0 05), and there was significant difference between the two groups in postoperative symptoms (P < 0 01), but there was no significant difference between the two groups (P < 0 05). There was no significant difference in postoperative curative effect between the two groups (P > 0.05), indicating that the two groups had the same postoperative curative effect. Conclusion 980nm semiconductor red laser vaporization of prostate can significantly improve the symptoms of lower urinary tract obstruction, and has the same short-term curative effect as TURP. Compared with TURP, it has simple operation and less bleeding during operation. It is a safe and effective minimally invasive operation for BPH due to its short time of indwelling catheter, short hospitalization time and quick recovery after operation.
