【摘要】：目的： 觀察經(jīng)尿道前列腺雙極等離子電切術(shù)(PKRP)聯(lián)合內(nèi)分泌治療對(duì)晚期前列腺癌伴膀胱出口梗阻(B00)的臨床療效。 方法： 收集2012年2月至2014年1月昆明醫(yī)科大學(xué)第二附屬醫(yī)院47例伴膀胱出口梗阻且行經(jīng)尿道前列腺雙極等離子電切術(shù)聯(lián)合內(nèi)分泌治療的晚期前列腺癌患者的臨床資料,記錄并通過統(tǒng)計(jì)學(xué)分析其治療前后國際前列腺癥狀評(píng)分(IPSS)、生活質(zhì)量評(píng)分(QOL)、殘余尿量(RU)、最大尿流率(Qmax)、血清前列腺特異性抗原(PSA)等指標(biāo)的變化程度和趨勢(shì)。 結(jié)果： 所有患者手術(shù)順利,并發(fā)癥少,術(shù)后所有患者排尿困難的癥狀均明顯改善,2例出現(xiàn)暫時(shí)性尿失禁,其余未見明顯尿失禁。8例骨痛患者骨痛明顯緩解,19例核素骨掃描示骨轉(zhuǎn)移灶明顯縮小,3例患者骨轉(zhuǎn)移灶消失。IPSS評(píng)分、Qmax、 QOL、RU較術(shù)前明顯改善,PSA較術(shù)前明顯下降。 結(jié)論： 經(jīng)尿道前列腺雙極等離子電切術(shù)聯(lián)合內(nèi)分泌治療能明顯提高伴膀胱出口梗阻的晚期前列腺癌患者的生活質(zhì)量,能顯著緩解膀胱出口梗阻癥狀,可作為晚期前列腺癌伴膀胱出口梗阻的患者的一種重要治療方法。
[Abstract]:Objective: to observe the clinical effect of transurethral bipolar plasma resection (PKRP) combined with endocrine therapy for advanced prostate cancer with bladder outlet obstruction (B00). Methods: from February 2012 to January 2014, the clinical data of 47 patients with advanced prostate cancer with bladder outlet obstruction treated by transurethral bipolar plasma resection combined with endocrine therapy in the second affiliated Hospital of Kunming Medical University were collected. The international prostatic symptom score (IPSS),) before and after treatment was recorded and analyzed statistically. (IPSS), quality of life score (QOL),) residual urine volume (RU),) maximum urinary flow rate (Qmax),) was recorded and analyzed before and after treatment. The change degree and trend of serum prostate specific antigen (PSA). Results: all the patients had smooth operation and few complications. The symptoms of dysuria were improved significantly in all patients after operation. Temporary urinary incontinence was found in 2 patients, and no obvious urinary incontinence was seen in the rest. Bone scan of 19 patients showed that bone metastases decreased significantly, and bone metastases disappeared in 3 patients. IPSS score, Qmax, QOL,RU and PSA were significantly improved compared with those before operation. Conclusion: transurethral bipolar plasma resection combined with endocrine therapy can significantly improve the quality of life of patients with advanced prostate cancer with bladder outlet obstruction, and can significantly alleviate the symptoms of bladder outlet obstruction. It can be used as an important treatment for advanced prostate cancer with bladder outlet obstruction.
