終末期腎臟病患者血清ucMGP及PIVKA-Ⅱ與橈動(dòng)脈鈣化的相關(guān)性研究

【摘要】:目的觀察終末期腎臟病患者血清非活性基質(zhì)Gla蛋白(uncarboxylated matrixGla protein,ucMGP)水平及橈動(dòng)脈鈣化情況,并探討二者之間的關(guān)系;通過(guò)凝血酶原前體蛋白(protein induced by vitamine K absence or antagonist-Ⅱ,PIVKA-Ⅱ)的檢測(cè)評(píng)估終末期腎臟病患者體內(nèi)維生素K狀態(tài);初步探討維生素K缺乏狀態(tài)對(duì)ucMGP及橈動(dòng)脈鈣化的影響。方法選取2012年6月至2013年5月安徽醫(yī)科大學(xué)附屬省立醫(yī)院腎臟內(nèi)科終末期腎臟病住院患者45例,行前臂橈動(dòng)脈-頭靜脈端端吻合術(shù)時(shí)取橈動(dòng)脈標(biāo)本,橈動(dòng)脈標(biāo)本均行鈣鹽染色,,判斷有無(wú)鈣化,并計(jì)算鈣化積分。另設(shè)對(duì)照組30例,來(lái)自我院健康體檢中心。所有參加者均抽取空腹血標(biāo)本檢查:采用血細(xì)胞分析儀測(cè)定血紅蛋白(hemoglobin,Hb)水平,采用全自動(dòng)生化分析儀測(cè)定血鈣(calcium,Ca)、血磷(phosphorus,P)、白蛋白(albumin,ALB),應(yīng)用電化學(xué)發(fā)光法測(cè)定血清全段甲狀旁腺素(intact parathyroid hormone,iPTH)、C反應(yīng)蛋白(Creactive protein,CRP)。采用酶聯(lián)免疫吸附技術(shù)(enzyme linked immunosorbentassay,ELISA)測(cè)定血清ucMGP及PIVKA-Ⅱ水平。 結(jié)果臨床血液指標(biāo)檢測(cè)病例組血Hb、Ca、ALB低于對(duì)照組(P<0.01),P、iPTH、CRP高于對(duì)照組(P<0.01)。病例組血清ucMGP為(198.96±93.96)nmol/L,對(duì)照組為(468.17±191.90)nmol/L,病例組低于對(duì)照組(P<0.01)。病例組血清PIVKA-Ⅱ?yàn)椋?.79±4.05)μg/L,對(duì)照組為(1.92±1.59)μg/L,病例組高于對(duì)照組(P<0.01)。病例組45例橈動(dòng)脈標(biāo)本中,鈣化均發(fā)生在血管中膜。無(wú)鈣化的28例(62.2%),輕度鈣化的12例(26.7%),重度鈣化的5例(11.1%)。橈動(dòng)脈無(wú)鈣化組、輕度鈣化組及重度鈣化組3組間比較血ucMGP、血P水平差異有統(tǒng)計(jì)學(xué)意義(P<0.01)。LSD法組間比較提示重度鈣化組血清PIVKA-Ⅱ與輕度鈣化組PIVKA-Ⅱ差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。Spearman相關(guān)分析提示病例組橈動(dòng)脈鈣化程度與ucMGP呈負(fù)相關(guān)(r=-0.64,P<0.05),與血清P、iPTH、CRP呈正相關(guān)。Pearson相關(guān)分析提示ucMGP與血清P呈負(fù)相關(guān)(rs=-0.31,P<0.05),血清ucMGP與PIVKA-Ⅱ無(wú)明顯相關(guān)性。 結(jié)論終末期腎臟病患者血清ucMGP低于正常人群,且與橈動(dòng)脈中膜鈣化相關(guān)。終末期腎臟病患者血清PIVKA-Ⅱ高于正常人群,該人群存在維生素K缺乏。終末期腎臟病患者血清ucMGP水平與PIVKA-Ⅱ水平無(wú)明顯相關(guān)性。
