【摘要】：目的探討女性早期尿道口惡性腫瘤保膀胱控尿性前尿道切除術(shù)后前尿道缺失采用會陰環(huán)形皮瓣一期吻合的可行性及有效性。方法納入2013年1月至2016年1月第二軍醫(yī)大學(xué)長海醫(yī)院女性尿道口早期惡性腫瘤患者29例;術(shù)前患者平均年齡(61.4±14.4)歲,臨床分期均為Ⅰ期,病變局限于前尿道的一半以內(nèi)。行保膀胱控尿性前尿道切除術(shù),腫瘤切除后,自會陰前庭向周圍游離環(huán)皮瓣約30~40mm長,與尿道近心殘端內(nèi)嵌式端端吻合。評估其臨床可行性及安全性。結(jié)果手術(shù)時(shí)間0.45~1.50h,平均(0.51±0.30)h。患者術(shù)后尿道口形狀接近生理結(jié)構(gòu),排尿通暢,最大尿流率12.8~33.0mL/s,平均(19.3±8.7)mL/s。一期成功率為100%,無尿道狹窄、尿失禁。隨訪12~48個(gè)月,平均(32.6±3.5)個(gè)月。1例鱗癌患者一期手術(shù)后2年出現(xiàn)左側(cè)腹股溝淋巴結(jié)轉(zhuǎn)移,術(shù)后3年肺部廣泛轉(zhuǎn)移死亡;1例鱗癌患者術(shù)后13個(gè)月腹股溝局部淋巴結(jié)腫大,給予髂腹股溝淋巴結(jié)清掃,術(shù)后病理報(bào)告為轉(zhuǎn)移性鱗癌。其余患者隨訪期間均未見復(fù)發(fā)。結(jié)論對女性早期尿道口惡性腫瘤保膀胱控尿性前尿道切除術(shù)后前尿道缺失采用會陰環(huán)形皮瓣一期吻合手術(shù)成功率高,安全有效。
