【摘要】：中年男性,腎移植術(shù)后服用潑尼松、環(huán)孢素、硫唑嘌呤及雷公藤多苷片等抗排斥治療,既往曾患新生隱球菌病(累及肺部及中樞神經(jīng)系統(tǒng)),氟康唑及伊曲康唑抗真菌治療后痊愈,后續(xù)潑尼松聯(lián)合嗎替麥考酚酯、他克莫司抗排斥。近4月出現(xiàn)反復(fù)發(fā)熱伴腹瀉,PET-CT提示全身多處淋巴結(jié)代謝增高,多次痰及尿液找抗酸桿菌陽(yáng)性,腹腔淋巴結(jié)活檢提示"鳥型胞內(nèi)分支桿菌假瘤",最終診斷為播散性鳥胞內(nèi)分枝桿菌復(fù)合體感染,經(jīng)治療后病情穩(wěn)定。
[Abstract]:A middle-aged man who received anti-rejection therapy such as prednisone, cyclosporine, azathioprine and tripterygium wilfordii polyglycosides after kidney transplantation, had a history of cryptococcosis (involving the lungs and central nervous system). Fluconazole and itraconazole were cured after antifungal therapy, followed by prednisone combined with motimicol ester, tacrolimus anti-rejection. In the last four months, repeated fever and diarrhea appeared. PET-CT indicated that the metabolism of multiple lymph nodes was increased, acid-fast bacilli were positive in sputum and urine many times, and abdominal lymph node biopsy suggested that "pseudotumor of Mycobacterium endocytogenes in birds". Finally, disseminated mycobacterium complex infection was diagnosed, and the disease was stable after treatment.
