【摘要】：目的探討3D高清腹腔鏡系統(tǒng)在泌尿外科領(lǐng)域的應(yīng)用價值。方法回顧性分析2014年3月~2016年9月我院應(yīng)用STORZ 3D高清腹腔鏡系統(tǒng)完成泌尿外科手術(shù)75例資料,包括后腹腔鏡腎部分切除術(shù)23例、根治性腎切除術(shù)17例、腎上腺腫物切除術(shù)5例、腎囊腫去頂術(shù)1例,經(jīng)腹腔途徑完全腹腔鏡腎輸尿管全長切除術(shù)2例,經(jīng)腹膜外途徑腹腔鏡根治性前列腺切除術(shù)19例,經(jīng)腹膜途徑腹腔鏡根治性膀胱切除術(shù)8例(尿流改道方式為回腸膀胱術(shù)7例,Studer原位新膀胱術(shù)1例)。結(jié)果 75例手術(shù)順利進(jìn)行,均獲成功。腎部分切除術(shù)手術(shù)時間76~206 min,術(shù)中出血10~200 ml;根治性腎切除術(shù)手術(shù)時間84~228 min,術(shù)中出血10~400 ml;腎上腺腫物切除術(shù)手術(shù)時間63~115 min,術(shù)中出血5~150 ml;腎囊腫去頂術(shù)手術(shù)時間53 min,術(shù)中出血20 ml;腎輸尿管全長切除術(shù)手術(shù)時間分別為393 min和165 min,術(shù)中出血分別為300 ml和50 ml;根治性前列腺切除術(shù)手術(shù)時間121~410 min,術(shù)中出血20~600 ml;根治性膀胱切除、回腸膀胱術(shù)手術(shù)時間220~561 min,術(shù)中出血150~600 ml;根治性膀胱切除、Studer原位新膀胱術(shù)手術(shù)時間288 min,術(shù)中出血200 ml。75例圍手術(shù)期無嚴(yán)重(Ⅲ~Ⅳ級)并發(fā)癥發(fā)生。結(jié)論 3D腹腔鏡有三維立體高清的畫面,解剖層次清晰,縫合操作相對容易,可以在臨床推廣應(yīng)用。
[Abstract]:Objective to explore the application value of 3 D high-definition laparoscopic system in urology. Methods from March 2014 to September 2016, 75 cases of urological surgery with STORZ 3D high-definition laparoscopic system were retrospectively analyzed, including 23 cases of retroperitoneal laparoscopic partial nephrectomy, 17 cases of radical nephrectomy and 5 cases of adrenalectomy. There were 1 case of renal cystectomy, 2 cases of complete laparoscopic nephroureterectomy, 19 cases of laparoscopic radical prostatectomy via extraperitoneal approach. Laparoscopic radical cystectomy via peritoneal approach was performed in 8 cases (7 cases underwent ileal cystectomy and 1 case underwent Studer in situ cystectomy). Results 75 cases were successfully operated. Partial nephrectomy operation time 76 ~ 206 min, Hemorrhage 10 ~ 200 ml; radical nephrectomy operative time 84 ~ 228 min, intraoperative bleeding 10 ~ 400 ml; adrenal tumor excision operative time 63115 min, intraoperative bleeding 5 ~ 150 ml; Operative time of renal cystectomy: operation time: 53 min, intraoperative hemorrhage 20 ml; total nephroureterectomy time: 393 min and 165 min, intraoperative bleeding: 300 ml and 50 ml;, respectively Operation time of radical prostatectomy: 121o410 min, intraoperative bleeding 200.600 ml; radical cystectomy, ileal cystectomy, 2200.561 min, intraoperative bleeding, 150-600 ml; Radical cystectomy and Studer orthotopic cystectomy were performed for 200 ml.75 patients with intraoperative bleeding.
