趨化因子受體4在腎透明細(xì)胞癌組織中的表達(dá)及臨床意義

【摘要】：目的：探討趨化因子受體4(CC chemokine receptor4, CCR4)在腎透明細(xì)胞癌(clear cell renal cell carcinoma, ccRCC)組織中的表達(dá)及其與臨床病理特征和預(yù)后之間的關(guān)系。方法：應(yīng)用免疫組織化學(xué)法檢測(cè)53例ccRCC和11例正常腎臟組織中CCR4的表達(dá)。結(jié)果：腎癌組織中CCR4表達(dá)陽(yáng)性率為52.83%(28/53),正常腎臟組織中CCR4陽(yáng)性率為18.2%(2/11),差異有統(tǒng)計(jì)學(xué)意義(χ2=4.392, P=.036)。CCR4表達(dá)與腫瘤分期(P=0.009),淋巴結(jié)轉(zhuǎn)移(P=0.003)及遠(yuǎn)處轉(zhuǎn)移(P=0.031)相關(guān)。CCR4是ccRCC患者發(fā)生淋巴結(jié)轉(zhuǎn)移唯一獨(dú)立的影響因素(P=0.014).總生存時(shí)間的影響因素分析,單因素分析顯示CCR4、腫瘤分期、淋巴結(jié)轉(zhuǎn)移與ccRCC患者總生存時(shí)間(overall survival, OS)相關(guān),多因素分析顯示CCR4(P=0.007)是ccRCC患者OS的唯一獨(dú)立預(yù)測(cè)指標(biāo)。Kaplan-Meier曲線提示CCR4高表達(dá)組的ccRCC患者OS短于CCR4高表達(dá)組(P=0.010)。結(jié)論：CCR4與腎透明細(xì)胞癌的惡性生物學(xué)行為有關(guān),可能成為判斷ccRCC預(yù)后指標(biāo)及潛在治療分子靶點(diǎn)。
[Abstract]:Objective: to investigate the expression of chemokine receptor 4 (CC chemokine receptor4, CCR4) in renal clear cell carcinoma (clear cell renal cell carcinoma, ccRCC) and its relationship with clinicopathological features and prognosis. Methods: immunohistochemical method was used to detect the expression of CCR4 in 53 cases of ccRCC and 11 cases of normal kidney. Results: the positive rate of CCR4 expression was 52.83% (28 / 53) in renal cell carcinoma and 18.2% (2 / 11) in normal kidney tissue. CCR4 expression was associated with tumor stage (P0. 009), lymph node metastasis (P0. 003) and distant metastasis (P0. 031). CCR4 was the only independent factor affecting lymph node metastasis in ccRCC patients (P0. 014). Univariate analysis showed that CCR4, tumor staging and lymph node metastasis were correlated with total survival time (overall survival, OS) of patients with ccRCC. Multivariate analysis showed that CCR4 (P0. 007) was the only independent predictor of OS in ccRCC patients. The Kaplan-Meier curve suggested that the OS of ccRCC patients with high expression of CCR4 was shorter than that of high expression of CCR4 (P0. 010). Conclusion: CCR4 is related to the malignant biological behavior of renal clear cell carcinoma and may be a prognostic index and a potential molecular target for the treatment of ccRCC.
