【摘要】：目的:通過觀察勃起功能障礙(ED)患者中血清計算游離睪酮(cFT)、睪酮分泌指數(shù)(TSI)及游離睪酮指數(shù)(FTI)的變化情況,探討cFT、TSI、FTI在ED伴雄激素缺乏診斷中的價值。方法:詳細(xì)詢問病史,填寫國際勃起功能指數(shù)問卷(IIEF-5),完成夜間陰莖勃起功能檢測(NPT),根據(jù)病史及NPT檢查結(jié)果納入實驗組及對照組,檢測150例NPT結(jié)果異常以"ED"為主訴的ED患者及25例NPT結(jié)果正常的健康婚檢者中血清總睪酮(TT)、黃體生成素(LH)、性激素結(jié)合球蛋白(SHBG)和血清白蛋白(ALB),通過公式計算cFT、生物可利用睪酮(Bio-T)、TSI、FTI。以cFT≤0.3 nmol/L、TSI≤2.8、FTI≤0.4為參考截點值,以TT≤11.5 nmol/L為診斷雄激素缺乏的標(biāo)準(zhǔn),計算出cFT、TSI、FTI的漏診率、誤診率及符合率。結(jié)果:以TT≤11.5 nmol/L為評估標(biāo)準(zhǔn),診斷20~40歲ED患者雄激素缺乏的符合率分別為cFT 90.8%、TSI 85.8%、FTI 80.8%;診斷20~40歲ED患者雄激素缺乏的漏診率分別為cFT 4.0%、TSI 33.3%、FTI 44.0%;診斷20~40歲ED患者雄激素缺乏的誤診率分別為cFT 10.5%、TSI9.4%、FTI 12.6%。Kappa值分別為:cFT 0.755、TSI 0.564、FTI 0.427,P均0.05。TT在不同年齡組ED患者中隨年齡下降但無統(tǒng)計學(xué)差異,cFT、Bio-T、TSI、FTI在不同年齡組ED患者中隨年齡下降且具有統(tǒng)計學(xué)差異;20~40歲ED患者血清TT、cFT、Bio-T、TSI、FTI在不同IIEF評分組的差別無統(tǒng)計學(xué)差異。結(jié)論:cFT對20~40歲ED患者伴有雄激素缺乏的檢測價值優(yōu)于TT、TSI及FTI。
[Abstract]:Objective: to investigate the value of cFT,TSI,FTI in the diagnosis of ED with androgen deficiency by observing the changes of serum free testosterone (cFT), testosterone secretion index (TSI) and free testosterone index (FTI) in patients with erectile dysfunction (ED). Methods: the history was inquired in detail, the international erectile function index questionnaire (IIEF-5) was filled out, and the night penile erectile function test (NPT), was completed. According to the history of the disease and the results of NPT examination, the patients were included in the experimental group and the control group. Serum total testosterone, (TT), luteinizing hormone, luteinizing globulin (SHBG) and serum albumin (ALB), were measured in 150 ED patients with abnormal NPT results, "ED" and 25 healthy subjects with normal NPT results.
