【摘要】：目的評價超選擇性栓塞治療有假性動脈瘤的高流量性陰莖異常勃起的技術(shù)與療效。方法 2006年6月—2012年9月我院介入科共收治有假性動脈瘤的高流量性陰莖異常勃起患者3例,采用明膠海綿進行超選擇插管栓塞,栓塞術(shù)后評價患者的陰莖勃起狀態(tài),并分別于術(shù)后3、6個月對患者進行彩色多普勒超聲隨訪,對勃起功能隨訪20~92個月,根據(jù)國際勃起功能指數(shù)5(IIEF5)評分標(biāo)準(zhǔn)進行評價。結(jié)果超選擇動脈插管及栓塞均獲得成功。12個月內(nèi)隨訪發(fā)現(xiàn)1例患者因過度栓塞出現(xiàn)輕度勃起功能障礙,IIEF5評分15分,2例患者勃起功能正常,評分22分和23分,隨訪期內(nèi)所有患者未再復(fù)發(fā)。結(jié)論超選擇栓塞對治療有假性動脈瘤的高流量性陰莖異常勃起安全、有效,正確超選擇靶血管及合適的栓塞劑用量是成功的關(guān)鍵。
[Abstract]:Objective to evaluate the technique and effect of superselective embolization in the treatment of high flow abnormal erectile penis with pseudoaneurysm. Methods from June 2006 to September 2012, 3 patients with high flow abnormal erection of penis with pseudoaneurysm were treated in our interventional department. Superselective embolization with gelatin sponge was used to evaluate the penile erectile status after embolization. The patients were followed up by color Doppler ultrasound at 3 months and 6 months after operation respectively. The erectile function was followed up for 20 ~ 92 months and evaluated according to the international erectile function index 5 (IIEF5) score. Results after 12 months of follow-up, one patient had mild erectile dysfunction due to excessive embolization, the IIEF5 score was 15, 2 patients had normal erectile function with 22 and 23 points. No recurrence occurred in all patients during the follow-up period. Conclusion Super-selective embolization is safe and effective in the treatment of high flow abnormal erection of penis with pseudoaneurysm. The key to success is the correct choice of target vessels and appropriate dosage of embolization agent.
