【摘要】：目的:1470nm半導(dǎo)體激光治療高危前列腺增生患者的臨床研究。方法:回顧性分析我院2015年7月-2017年2月應(yīng)用1470nm半導(dǎo)體激光治療高危BPH患者61例臨床資料.比較手術(shù)前后血紅蛋白(Hb)的變化,生活質(zhì)量評分(QOL),國際前列腺癥狀評分(IPSS),殘余尿量(RU),最大尿流率(Qmax)等指標(biāo)。結(jié)果:全部手術(shù)成功,手術(shù)時間10-78min,平均為42min,61例患者均能自主排尿,Hb較術(shù)前輕度下降,未出現(xiàn)大出血、TURS及尿失禁,2例全麻患者術(shù)后一周內(nèi)出現(xiàn)墜積性肺炎,應(yīng)用敏感抗生素2周后治愈。61例患者獲隨訪3個月,58例排尿情況較術(shù)前明顯改善,有3例患者術(shù)后三月左右再次出現(xiàn)尿潴留。結(jié)論:應(yīng)用1470nm半導(dǎo)體激光治療高危前列腺增生患者具有出血少、創(chuàng)傷小、安全性高等優(yōu)點,高危BPH患者圍手術(shù)期力求個性化處理,只要加強圍手術(shù)期的管理,積極治療伴隨癥,手術(shù)治療是安全可行的。
[Abstract]:Objective: to study the clinical effect of 1470nm semiconductor laser in the treatment of high risk benign prostatic hyperplasia (BPH). Methods: the clinical data of 61 high risk BPH patients treated with 1470nm semiconductor laser from July 2015 to February 2017 were retrospectively analyzed. The changes of hemoglobin (Hb) before and after operation, quality of life score (QOL), international prostate symptom score (IPSS), residual urine volume (RU), maximum urinary flow rate (Qmax) were compared. Results: all the operations were successful. The operation time was 10-78 mins, the average time was 42 mins, 61 patients were able to urinate autonomously. The Hb decreased slightly, there was no massive bleeding, TURS and urinary incontinence occurred in 2 patients with general anesthesia within one week after operation. 61 patients were followed-up for 3 months, 58 patients had better urination than before, and 3 patients had urinary retention about 3 months after operation. Conclusion: the application of 1470nm semiconductor laser in the treatment of high risk patients with benign prostatic hyperplasia has the advantages of less bleeding, less trauma and high safety. The perioperative management of high risk BPH patients is to be individualized, so long as the management of perioperative period is strengthened and the patients with prostatic hyperplasia are treated actively. Surgical treatment is safe and feasible.
