腦特異性血管生成抑制因子1在膀胱尿路上皮癌中的表達(dá)及臨床相關(guān)性研究

【摘要】:目的:本研究旨在明確BAI-1基因在膀胱正常組織和膀胱尿路上皮癌組織中的表達(dá)情況及其對(duì)膀胱尿路上皮癌實(shí)體腫瘤新生血管的作用;分析BAI-1基因表達(dá)與臨床各病理參數(shù)之間的相關(guān)性,探討B(tài)AI-1基因在膀胱尿路上皮癌的發(fā)生、發(fā)展中的作用,以期為膀胱尿路上皮癌的診治提供幫助。方法:采用逆轉(zhuǎn)錄-聚合酶鏈反應(yīng)(Reverse transcription-polymerase chain reaction,RT-PCR)檢測20例膀胱正常組織和20例膀胱尿路上皮癌組織的BAI-1 mRNA的含量。在免疫組織化學(xué)SP法檢測BAI-1表達(dá)情況的方法中,肌層浸潤性膀胱尿路上皮癌、非肌層浸潤性膀癌和正常膀胱組織分別為93例、39例和30例,同時(shí)檢測腫瘤相關(guān)巨噬細(xì)胞(TAMs,即CD68+)及微血管密度(MVD,即CD34+)在上述組織中的表達(dá)。應(yīng)用蛋白質(zhì)印跡法(Western Blot)檢測膀胱正常組織、非肌層浸潤性和肌層浸潤性膀胱尿路上皮癌中各自BAI-1蛋白含量的多少;仡櫺苑治132例患者的BAI-1基因表達(dá)的高低與所納入的膀胱尿路上皮癌患者的各臨床及病理因素的關(guān)系。結(jié)果:1、BAI-1 mRNA在膀胱正常組織中的表達(dá)明顯高于癌組織,其差異具有統(tǒng)計(jì)學(xué)意義(p0.05)。2、BAI-1蛋白在膀胱正常組織中的表達(dá)明顯高于癌組織,其差異具有統(tǒng)計(jì)學(xué)意義(p0.05)。3、BAI-1在膀胱尿路上皮癌中的表達(dá)與TAMs(CD68+)和MVD(CD34+)的表達(dá)均成負(fù)相關(guān),而TAMs和MVD在膀胱尿路上皮癌中的表達(dá)則呈現(xiàn)出正相關(guān)關(guān)系,各自差異均具有統(tǒng)計(jì)學(xué)意義(p0.05)。4、BAI-1在膀胱尿路上皮癌中的表達(dá)的高低與年齡、性別、腫瘤大小、數(shù)目、吸煙及腫瘤的轉(zhuǎn)移無關(guān)(p0.05),但是與腫瘤的分期、分級(jí)有關(guān),(p0.05)。BAI-1的低表達(dá)對(duì)膀胱尿路上皮癌患者的總生存和無復(fù)發(fā)生存具有顯著影響(log-rank p0.05)。結(jié)論:1、BAI-1 mRNA及其BAI-1基因所表達(dá)的BAI-1蛋白在正常膀胱組織中表達(dá)陽性率比膀胱尿路上皮癌組織中表達(dá)的高;且BAI-1比到達(dá)的高低與腫瘤的分期分級(jí)有關(guān)。2、BAI-1基因低表達(dá)與TAMs(CD68+)的高表達(dá)和與膀胱尿路上皮癌的高分期、高分級(jí)和復(fù)發(fā)密切關(guān)聯(lián),提示BAI-1的低表達(dá)與TAMs(CD68+)的高表達(dá)的負(fù)相關(guān)在膀胱尿路上皮癌的發(fā)生、發(fā)展中起到一定的作用。
[Abstract]:Objective: to investigate the expression of BAI-1 gene in bladder normal tissues and bladder urothelial carcinoma tissues and its role in the neovascularization of solid tumors of bladder urothelial carcinoma. To analyze the relationship between the expression of BAI-1 gene and the clinicopathological parameters, and to explore the role of BAI-1 gene in the occurrence and development of bladder urothelial carcinoma, in order to provide help for the diagnosis and treatment of bladder urothelial carcinoma. Methods: reverse transcription-polymerase chain reaction (Reverse transcription-polymerase chain reaction,RT-PCR) was used to detect the content of BAI-1 mRNA in 20 cases of normal bladder and 20 cases of bladder urothelial carcinoma. In the immunohistochemical SP method, the expression of BAI-1 was detected in 93 cases of bladder epithelial carcinoma, 30 cases of non-myometrial bladder carcinoma and 30 cases of normal bladder tissue, respectively, in which there were 93 cases of invasive bladder epithelial carcinoma, 30 cases of non-muscular invasive bladder carcinoma and 30 cases of normal bladder tissue. The expression of tumor associated macrophages (TAMs, CD68) and microvessel density (MVD, (CD34) was also detected. Western blot (Western Blot) was used to detect the content of BAI-1 protein in bladder normal tissue, non-myometrial infiltrating bladder and myometrial invasive bladder urothelial carcinoma. The relationship between the expression of BAI-1 gene and the clinical and pathological factors of bladder urothelial carcinoma was analyzed retrospectively. Results the expression of BAI-1 mRNA in normal bladder tissues was significantly higher than that in normal bladder tissues, and the difference was statistically significant (p0.05) .2the expression of BAI-1 protein in normal bladder tissues was significantly higher than that in cancer tissues. The difference was statistically significant (p0.05) .3BAI-1 expression in bladder urothelial carcinoma was negatively correlated with the expression of TAMs (CD68) and MVD (CD34), while the expression of TAMs and MVD in bladder urothelial carcinoma was positively correlated. The differences were statistically significant (p0.05). 4 the expression of BAI-1 in bladder urothelial carcinoma was not related to age, sex, tumor size, number, smoking and metastasis (p0.05). The low expression of (p0. 05). BAI-1 had a significant effect on the overall survival and recurrence free survival of patients with bladder urothelial carcinoma (log-rank p0. 05). Conclusion the positive rate of BAI-1 protein expression in normal bladder tissue is higher than that in bladder urothelial carcinoma tissue. The low expression of BAI-1 gene was associated with the high expression of TAMs (CD68) and the high grade, high grade and recurrence of bladder urothelial carcinoma. The results suggest that the low expression of BAI-1 and the high expression of TAMs (CD68) play a role in the occurrence and development of bladder urothelial carcinoma.
