MRU、IVU、CTU在泌尿系統(tǒng)疾病檢查中的對(duì)照研究

【摘要】:目的評(píng)價(jià)高場(chǎng)強(qiáng)磁共振泌尿系水成像技術(shù)(MRU)、靜脈尿路造影技術(shù)(IVU)與多層螺旋CT泌尿系成像技術(shù)(CTU)在泌尿系統(tǒng)疾病檢查中的應(yīng)用價(jià)值。方法回顧性分析140例經(jīng)手術(shù)病理和臨床隨訪證實(shí)為泌尿系統(tǒng)疾病患者的影像學(xué)資料。行MRU檢查101例,行IVU檢查91例,行CTU檢查38例。其中同時(shí)行IVU及MRU檢查者68例,同時(shí)行IVU及CTU檢查者29例,同時(shí)行CTU及MRU檢查者11例,有7例患者同時(shí)行3項(xiàng)檢查。結(jié)果與金標(biāo)準(zhǔn)診斷結(jié)果相比,7種組合的檢查方法對(duì)泌尿系統(tǒng)疾病的診斷一致率差異有統(tǒng)計(jì)學(xué)意義(P0.05),其中IVU檢查一致率最低,僅為38.46%,而MRU與CTU檢查的一致率分別為78.22%及86.84%,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),CTU聯(lián)合MRU檢查與金標(biāo)準(zhǔn)的一致率為100.00%,高于單純MRU檢查的一致率78.22%,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 IVU價(jià)格低廉,適合基層醫(yī)院和篩查病變。MRU無(wú)創(chuàng)、無(wú)射線損害,適合孕婦、兒童及造影劑過(guò)敏者,并且可作為首選或IVU之后的進(jìn)一步檢查手段。MRU結(jié)合CTU可提高診斷準(zhǔn)確率,但CTU價(jià)格昂貴,患者接受射線量大,不宜作為常規(guī)檢查。
[Abstract]:Objective to evaluate the value of high field magnetic resonance urography (MRU),) intravenous urography (IVU) and multislice spiral CT urography (CTU) in the examination of urinary system diseases. Methods the imaging data of 140 patients with urinary system diseases confirmed by operation, pathology and clinical follow-up were retrospectively analyzed. MRU was performed in 101 cases, IVU in 91 cases and CTU in 38 cases. Among them, 68 cases were examined by IVU and MRU, 29 cases by IVU and CTU, 11 cases by CTU and MRU, and 7 cases by 3 items at the same time. Results compared with the diagnostic results of gold standard, there was a significant difference in the diagnostic consistency rate of the seven combined methods for urinary system diseases (P0.05), in which the consistent rate of IVU was the lowest. The consistency rate of MRU and CTU was 78.22% and 86.84, respectively. There was no significant difference (P0.05). The concordance rate between MRU and gold standard was 100.000.This was higher than that of MRU alone (78.22%). The difference was statistically significant (P0.05). Conclusion IVU is low in price, suitable for primary hospital and non-invasive screening. MRU is suitable for pregnant women, children and those with contrast medium allergy, and can be used as the first choice or further examination method after IVU. MRU combined with CTU can improve the diagnostic accuracy. However, CTU is expensive, patients receive large amounts of radiation, should not be used as a routine examination.
