【摘要】：目的分析乳頭狀腎細(xì)胞癌的臨床特點(diǎn)及病理特征。方法回顧性分析6例乳頭狀腎細(xì)胞癌患者的臨床資料,并與同期住院腎腫瘤(透明細(xì)胞癌、錯構(gòu)瘤、其他類型腎細(xì)胞癌)患者進(jìn)行比較。結(jié)果乳頭狀腎細(xì)胞癌患者年齡41~68歲,均為男性。就診時(shí)4例無癥狀,2例有肉眼血尿。腫瘤最大直徑平均4.6 cm,未見多中心病灶。TNM分期T1aN0M04例,T1bN0M01例,T2N0M01例。術(shù)后病理腫瘤組織呈乳頭狀或管狀結(jié)構(gòu)排列,乳頭中心及間質(zhì)內(nèi)有泡沫細(xì)胞浸潤,有明顯出血壞死。病理分型Ⅰ型4例,Ⅱ型2例。Fuhrman分級Ⅱ級3例,Ⅲ級3例。結(jié)論乳頭狀腎細(xì)胞癌好發(fā)于男性,影像學(xué)檢查不具備惡性腫瘤特征,其確診有賴于病理和免疫組織化學(xué)檢查;臨床分期均為I或Ⅱ期,就診時(shí)無一例出現(xiàn)遠(yuǎn)處轉(zhuǎn)移。其臨床表現(xiàn)與其他腎細(xì)胞癌相似,但影像學(xué)表現(xiàn)、病理形態(tài)及生物學(xué)行為表現(xiàn)均與其他腎細(xì)胞癌有所不同。
[Abstract]:Objective to analyze the clinical and pathological features of papillary renal cell carcinoma. Methods the clinical data of 6 patients with papillary renal cell carcinoma were retrospectively analyzed and compared with those of hospitalized renal tumors (clear cell carcinoma, hamartoma, other types of renal cell carcinoma). Results the age of patients with papillary renal cell carcinoma was 41 ~ 68 years old, all of them were male. There were 4 cases of asymptomatic hematuria in 2 cases. The mean maximum diameter of the tumor was 4.6 cm. There were no multicentric lesions. TNM staging was performed in T _ 1bN _ 0M _ 01 and T _ 2N _ 0M _ 01. The pathological tumor tissues were arranged in papillary or tubular structure, foam cells were infiltrated in the center of papilla and interstitium, and hemorrhage and necrosis were observed.
