【摘要】：射精管梗阻(EDO)是梗阻性因素引發(fā)男性不育的原因之一,也是可經(jīng)外科手術(shù)治愈的男性不育癥之一,在所有男性不育癥中占1%~5%。相比后天性因素先天性發(fā)育異常是EDO發(fā)病最常見的原因。EDO臨床表現(xiàn)多樣,典型的精液呈現(xiàn)"四低"特征,經(jīng)直腸超聲是EDO篩查的重要影像學(xué)檢查和手術(shù)治療中的引導(dǎo),磁共振檢查提供生殖系統(tǒng)更清晰的影像依據(jù)。經(jīng)尿道射精管切除術(shù)(TURED)為代表的經(jīng)尿道微創(chuàng)手術(shù)是治療EDO的經(jīng)典術(shù)式,而各式經(jīng)尿道精道鏡技術(shù)的應(yīng)用成為治療EDO的趨勢,最新出現(xiàn)的精囊輸精管軟鏡或輸精管鏡技術(shù)有望進(jìn)一步改進(jìn)EDO的診治。
[Abstract]:Ejaculatory duct obstruction (EDO) is one of the causes of male infertility caused by obstructive factors. It is also one of the male infertility that can be cured by surgery. It accounts for 1 / 5 of all male infertility. Compared with acquired factors, congenital dysplasia is the most common cause of EDO. EDO is the most common cause of EDO. The typical semen shows "four low" characteristics. Transrectal ultrasound is an important imaging examination of EDO screening and guidance in surgical treatment. Magnetic resonance imaging provides a clearer imaging basis for the reproductive system. Transurethral minimally invasive surgery, represented by transurethral ejaculatory duct resection (TURED), is the classic procedure for the treatment of EDO, and the application of various transurethral endoscopy techniques has become the trend in the treatment of EDO. The newly developed soft or vas deferens techniques are expected to further improve the diagnosis and treatment of EDO.
