【摘要】：目的探討多發(fā)性肌層浸潤性膀胱癌中腫瘤發(fā)生部位與盆腔淋巴結(jié)轉(zhuǎn)移和分布情況，以為此類腫瘤制定更為合理的淋巴結(jié)清掃方案。 方法回顧性分析122例多發(fā)性膀胱癌并行根治性膀胱切除患者的臨床病例，男88例，女34例，平均年齡61歲。術(shù)后病理提示均為肌層浸潤性膀胱癌。依據(jù)大體及病理標本將腫瘤發(fā)生部位分為膀胱底部組、頸口組、側(cè)壁組和頂后壁組；盆腔淋巴結(jié)清掃區(qū)域分為閉孔組、髂內(nèi)組、髂外組、髂總組及骶前組。統(tǒng)計分析膀胱內(nèi)不同發(fā)生部位的腫瘤其各組淋巴結(jié)轉(zhuǎn)移的陽性率。 結(jié)果膀胱底部組共34例，淋巴結(jié)轉(zhuǎn)移率由高到低依次為閉孔、髂外、髂總、髂內(nèi)、骶前，髂總陽性率高于余下三組（P=0.013）；頸口組共29例，淋巴結(jié)轉(zhuǎn)移率依次為閉孔、髂外、髂內(nèi)、髂總、骶前，骶前陽性率高于其他三組（P=0.037）；側(cè)壁組與頂后壁組分別為27例與32例，淋巴結(jié)轉(zhuǎn)移率依次為閉孔、髂外、髂內(nèi)、髂總、骶前；總陽性淋巴結(jié)檢出率由高到低為：閉孔（16.8%）、髂外（12.7%）、髂內(nèi)（11.3%）、髂總（4.1%）、骶前（2.5%）；組間比較差異有統(tǒng)計學(xué)意義（P＜0.01）。 結(jié)論閉孔組淋巴結(jié)陽性率高于其余各組，應(yīng)作為常規(guī)清掃區(qū)域；膀胱底部腫瘤易發(fā)生髂總淋巴結(jié)轉(zhuǎn)移，頸口受侵后骶前淋巴結(jié)陽性率增高，，清掃區(qū)域應(yīng)向上擴大；在多發(fā)性肌層浸潤性膀胱癌中，不同部位腫瘤有其主要轉(zhuǎn)移區(qū)域，術(shù)中應(yīng)注意對該區(qū)域淋巴結(jié)重點清掃。
[Abstract]:Objective To investigate the location of tumor and the distribution of pelvic lymph node metastasis in multiple myometrial invasive bladder cancer (MIBC), so as to make a more reasonable lymph node dissection plan for this kind of tumor.
Methods The clinical data of 122 patients with multiple bladder cancer undergoing radical cystectomy were retrospectively analyzed, including 88 males and 34 females, with an average age of 61 years. The dissection area was divided into obturator group, internal iliac group, external iliac group, common iliac group and presacral group.
Results The total positive rate of lymph node metastasis in the bladder floor group was higher than that in the other three groups (P = 0.013), and that in the neck orifice group was higher than that in the other three groups (P = 0.037). The lymph node metastasis rates were obturator, external iliac, internal iliac, total iliac and presacral in turn; the total positive lymph node detection rates from high to low were obturator (16.8%), external iliac (12.7%), internal iliac (11.3%), total iliac (4.1%) and anterior sacral (2.5%).
Conclusion The positive rate of lymph nodes in obturator group is higher than that in other groups and should be used as a routine clearance area. Attention should be paid to key lymph nodes in this area.
