【摘要】：目的:探討血液凈化治療聯(lián)合濟(jì)生腎氣丸對尿毒癥患者蛋白質(zhì)和能量貯備的影響。方法:124例尿毒癥患者隨機(jī)分為對照組(n=60)和試驗(yàn)組(n=64)。對照組患者給予血液凈化治療,250~300 ml/min,每次4 h,每周3次;試驗(yàn)組患者在對照組治療的基礎(chǔ)上給予濟(jì)生腎氣丸9 g,每日2次。兩組療程均為12個月。觀察兩組患者治療前后血紅蛋白(Hb)、血尿素氮(BUN)、丙氨酸轉(zhuǎn)氨酶(ALT)、白蛋白(ALB)、血清轉(zhuǎn)鐵蛋白(TF)和血漿纖維連接蛋白(FN),收縮壓、舒張壓和平均動脈壓,體質(zhì)量指數(shù)(BMI)、皮下脂肪厚度和上臂肌肉周徑等情況。結(jié)果:治療后,兩組患者Hb、TF、FN水平均顯著高于治療前,且試驗(yàn)組高于對照組;ALT、ALB、BUN水平和收縮壓、舒張壓、平均動脈壓均顯著低于治療前,且試驗(yàn)組低于對照組;對照組患者BMI、皮下脂肪厚度和上臂肌肉周徑均顯著低于治療前和試驗(yàn)組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);試驗(yàn)組患者BMI、皮下脂肪厚度和上臂肌肉周徑與治療前比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:在血液凈化治療的基礎(chǔ)上給予濟(jì)生腎氣丸可改善患者體內(nèi)微環(huán)境,提高機(jī)體免疫功能,降低炎癥引起的代謝加快,改善患者蛋白質(zhì)和能量貯備,有助于提高其生存質(zhì)量。
[Abstract]:Objective: to investigate the effect of blood purification therapy combined with Jisheng Shenqi pills on protein and energy storage in uremic patients. Methods 124 patients with uremia were randomly divided into control group (n = 60) and experimental group (n = 64). The patients in the control group were treated with blood purification therapy of 250 ml / min for 4 h, 3 times a week, and the patients in the experimental group were given 9 g of Jisheng Shenqi pills on the basis of the treatment in the control group, twice a day. The course of treatment was 12 months in both groups. Serum transferrin (TF) and plasma fibronectin (FN), systolic blood pressure, diastolic blood pressure and mean arterial pressure were observed before and after treatment with hemoglobin (Hb), blood urea nitrogen, (BUN), alanine aminotransferase, (ALT), albumin (ALB), and plasma fibronectin (FN),. Body mass index (BMI),) subcutaneous fat thickness and upper arm muscle circumference. Results: after treatment, the levels of HbtbtfN in both groups were significantly higher than those before treatment, and the levels of ALB bun, systolic blood pressure, diastolic blood pressure and mean arterial pressure in the experimental group were significantly lower than those in the control group, and those in the experimental group were lower than those in the control group. The BMI, subcutaneous fat thickness and muscle circumference of upper arm in control group were significantly lower than those before treatment and experimental group (P0.05), the BMI, thickness of subcutaneous fat and circumference of upper arm muscle in test group were significantly lower than those before treatment (P0.05). The difference was not statistically significant (P0.05). Conclusion: on the basis of blood purification therapy, Jisheng Shenqi pills can improve the microenvironment, improve the immune function, decrease the metabolism caused by inflammation, improve the protein and energy storage, and improve the quality of life.
