【摘要】：目的探討自制單孔后腹腔鏡腎蒂淋巴管結扎術治療乳糜尿的安全性及臨床療效。方法回顧性分析2013年2月-2016年3月接受單孔后腹腔鏡腎蒂淋巴管結扎術或三孔后腹腔鏡腎蒂淋巴管結扎術治療乳糜尿的34例患者臨床資料,其中單孔組(16例)和三孔組(18例)。兩組患者的年齡、體質指數(shù)(BMI)比較差異無統(tǒng)計學意義(P0.05)。比較兩組的手術時間、術中出血量、術后引流管留置天數(shù)、術后住院天數(shù)、術后疼痛評分和外觀滿意度評分等指標。結果所有手術均獲成功,無中轉開放手術。單孔組平均手術時間長于三孔組[(132.4±21.6)vs(102.3±16.1)min,P0.05],兩組在術中出血量、術后拔管時間和術后住院天數(shù)方面比較差異無統(tǒng)計學意義,但單孔組術后滿意度評分、疼痛評分優(yōu)于三孔組。結論自制單孔后腹腔鏡腎蒂淋巴管結扎術治療乳糜尿是一種安全有效、經(jīng)濟、微創(chuàng)的方法,術后疼痛較輕,美容效果更好。
[Abstract]:Objective to investigate the safety and clinical efficacy of a self-made single hole laparoscopic renal pedicle lymphatic ligation in the treatment of chyluria. Methods the clinical data of 34 cases of chyluria treated by laparoscopic renal pedicle ligation or three hole laparoscopic renal pedicle lymphatic ligation in March, -2016, February 2013, were retrospectively analyzed. 6 cases and three hole group (18 cases). The age of the two groups and the body mass index (BMI) were not statistically significant (P0.05). The operation time of the two groups, the amount of intraoperative bleeding, the number of postoperative drainage tube indwelling days, the postoperative hospital days, the postoperative pain score and the appearance satisfaction score were measured. All the results were successful and no transfer open operation. The average operation time of the single hole group was longer than that of the three foramen Group [(132.4 + 21.6) vs (102.3 + 16.1) min), P0.05], the amount of bleeding in the two groups, the postoperative extubation time and the postoperative hospital days were not statistically significant, but the postoperative satisfaction score of the single hole group was better than the three hole group. Conclusion the self-made single hole laparoscopic renal pedicle lymphatic ligation Treatment of chyluria is a safe, effective, economical, minimally invasive method with less postoperative pain and better cosmetic results.
