特發(fā)性膜性腎病患者血清中小麥蛋白抗體的水平變化及意義

【摘要】:目的探討特發(fā)性膜性腎病(IMN)患者血清中小麥蛋白抗體的水平變化及意義。方法運用液氮研磨丙酮提取法粗提取小麥總蛋白,以此蛋白作為抗原,采用ELISA雙抗體間接法檢測40例IMN患者(IMN組)、34例其他腎小球腎炎患者(其他腎炎組)和40例健康人(對照組)血清中小麥蛋白特異性抗體IgG、IgA和IgE水平,并對患者進行食物過敏情況問卷調查,觀察小麥蛋白特異抗體水平明顯變化患者的臨床特征。結果 IMN組血清中小麥蛋白特異性IgG水平低于對照組(P0.01)、其他腎炎組(P0.05);其他腎炎組與對照組比較,P㧐0.05;各組血清中小麥蛋白特異性IgA水平比較,P0.05;各組血清中均未檢測到小麥蛋白特異性IgE。與其他腎炎組比較,IMN組血清肌酐、24 h尿蛋白定量升高,血漿白蛋白降低(P均0.05);兩組e GFR比較,P㧐0.05。20例血清中小麥蛋白特異性IgG抗體水平明顯降低(低于中位數)的IMN患者中12例尿蛋白大于5 g/24 h,且血清白蛋白水平20.0 g/L。結論 IMN患者血清中小麥蛋白特異性IgG升高,IgA、IgE無變化,IgG可能與IMN發(fā)生、發(fā)展有關。
[Abstract]:Objective to investigate the changes and significance of wheat protein antibody in patients with idiopathic membranous nephropathy (IMN). Methods Total protein of wheat was extracted by liquid nitrogen grinding acetone extraction method, and the protein was used as antigen. The levels of IgGA and IgE in serum of 40 IMN patients (IMN group), 34 other glomerulonephritis patients (other glomerulonephritis group) and 40 healthy people (control group) were detected by Elisa double antibody indirect method. A questionnaire survey was conducted to observe the clinical characteristics of wheat protein specific antibodies. Results the serum specific IgG level of wheat protein in IMN group was lower than that in control group (P0.01), other nephritis group (P0.05), other nephritis group compared with control group (P0.05), serum wheat protein specific IgA level in each group compared with control group (P0.05), and no detectable in serum of each group. Wheat protein specific IgE. Compared with other glomerulonephritis group, the serum creatinine in IMN group was significantly higher than that in other nephritis groups. Plasma albumin was decreased (P 0.05), and serum albumin level was 20.0 g / L in 12 IMN patients whose serum wheat protein specific IgG antibody level was significantly lower (lower than the median) than that in the control group (P 0.05), and the serum albumin level was 20.0 g / L in 12 patients with IMN whose serum albumin level was 20.0 g / L, which was more than 5 g / 24 h. Conclusion the serum specific IgG of wheat protein in IMN patients has no change in IgA and IgE, which may be related to the occurrence and development of IMN.
