【摘要】：腸道菌群失調(diào)是中、晚期慢性腎臟病(chronic kidney disease,CKD)進(jìn)展至尿毒癥的高危因素之一,其特征就是益生菌減少、能產(chǎn)生尿毒素的條件致病菌(產(chǎn)尿素酶菌群、產(chǎn)內(nèi)毒素菌群以及產(chǎn)代謝毒素菌群)增多。"腸-腎軸(gut-kidney axis)"學(xué)說(shuō)和"慢性腎臟病-結(jié)腸軸(chronic kidney disease-colonic axis)"概念的核心觀點(diǎn)認(rèn)為,失調(diào)的腸道菌群通過(guò)加劇尿毒素蓄積和誘導(dǎo)全身性微炎癥反應(yīng)等途徑加重腎臟的損傷。初步的臨床試驗(yàn)和動(dòng)物實(shí)驗(yàn)的結(jié)果顯示,以乳酸菌和雙歧桿菌制劑為代表的益生菌和以菊粉、低聚半乳糖為代表的益生元,還有魯比前列酮、活性炭吸附劑等都可以針對(duì)CKD患者體內(nèi)失調(diào)的腸道菌群,通過(guò)減輕尿毒素蓄積和抑制全身性微炎癥反應(yīng)等途徑而改善腎功能。不僅如此,一些經(jīng)口服或腸道給藥的單味中藥、中藥復(fù)方還有中藥提取物(大黃素等)在一定程度上也能調(diào)節(jié)腸道細(xì)菌,保護(hù)腸道上皮屏障,減輕尿毒素蓄積,延緩CKD進(jìn)展。其中,大黃類經(jīng)方——大黃甘草湯(濃縮顆粒TJ-84)改善腎衰竭動(dòng)物模型體內(nèi)尿毒素蓄積的靶點(diǎn)不是腎臟,而可能是"腸道菌群所啟動(dòng)的腸-腎軸"。基于此,針對(duì)中、晚期CKD患者腸組織中腸道菌群相關(guān)的病理因素(緊密連接蛋白、輔助性和調(diào)節(jié)性T細(xì)胞等)的干預(yù)性研究可能是今后的重點(diǎn)發(fā)展方向之一。
[Abstract]:Intestinal dysbacteriosis is one of the high risk factors for advanced chronic kidney disease (chronic kidney disease) to uremia, characterized by the reduction of probiotics and the production of urinary toxin (urease producing bacteria). The number of endotoxin producing bacteria and metabolic toxin producing bacteria were increased. " The core of the theory of "gut-kidney axis" and the concept of "chronic kidney disease-colon axis (chronic kidney disease-colonic axis)" suggests that the dysfunctional intestinal flora exacerbates renal damage by increasing the accumulation of urotoxin and inducing systemic microinflammation. Preliminary clinical and animal trials showed that probiotics represented by lactic acid bacteria and bifidobacterium, probiotics represented by chrysanthemum and galactose, and rubiprost, Activated carbon adsorbent can improve renal function by reducing accumulation of urinary toxin and inhibiting systemic microinflammation. Not only that, some oral or intestinal administration of single Chinese medicine, traditional Chinese medicine compound and traditional Chinese medicine extract (emodin and so on) can also regulate intestinal bacteria to some extent, protect intestinal epithelial barrier, reduce the accumulation of urotoxin, delay the progress of CKD. Among them, rhubarb meridian prescription-rhubarb Licorice decoction (concentrated granule TJ-84) can improve the accumulation of urinary toxin in animal model of renal failure not as kidney, but as "enterorenal axis initiated by intestinal flora". Therefore, the interventional study of intestinal microflora related to intestinal microflora (tight junction protein, helper T cells, etc.) may be one of the important development directions in the future.
