vWF、PAI-1在糖尿病大鼠腎組織的表達及丹紅注射液、二甲雙胍的干預研究

【摘要】:目的:糖尿病是一種全身的慢性代謝性疾病,其發(fā)病率逐年增高。糖尿病腎病(DN)是糖尿病最常見和嚴重的微血管并發(fā)癥,也是導致終末期腎臟病的一個重要原因。DN的發(fā)生發(fā)展是多因素綜合作用的結(jié)果,與糖代謝紊亂、血流動力學、細胞因子、氧化應激、炎癥反應、血液流變學等因素密切相關(guān)。血液流變學改變是糖尿病時微循環(huán)障礙的主要原因,在DN發(fā)展過程中起重要作用。血管性假血友病因子(vWF)、纖溶酶原激活物抑制劑-1(PAI-1)能反映機體的凝血、纖溶功能,參與DN的發(fā)生、發(fā)展。vWF是血管內(nèi)皮細胞損傷的標志物。vWF水平升高促進了血液的高凝狀態(tài)。PAI-1是纖溶系統(tǒng)的主要調(diào)控因子,能夠抑制纖溶活性。丹紅注射液的主要功能為活血化瘀,二甲雙胍是一種被廣泛應用的有效安全的降糖藥,,同時具有調(diào)節(jié)血脂,降低血小板聚集、改善血流變的作用。丹紅注射液、二甲雙胍可能能夠延緩DN的進展,但其具體機制尚待進一步研究。本研究觀察丹紅注射液、二甲雙胍干預前后糖尿病大鼠血清vWF水平及腎組織中vWF、PAI-1表達的變化,進一步探討DN發(fā)病機制及丹紅注射液、二甲雙胍的腎臟保護作用。 方法:48只雄性SD大鼠隨機分為2組:正常對照組(A組)10只、實驗組38只。實驗組大鼠均按55mg/kg劑量一次性腹腔注射鏈脲佐菌素(STZ),72小時后測血糖值>16.7mmol/L,持續(xù)三天,作為糖尿病大鼠模型建立的標準。把實驗組大鼠隨機分為三組:糖尿病組(B組)14只、丹紅干預組(C組)12只、二甲雙胍干預組(D組)12只。C組按2ml/kg·d劑量腹腔注射丹紅注射液,D組給予二甲雙胍300mg/kg·d灌胃給藥,A和B組分別給予腹腔注射相應劑量的生理鹽水。干預為期8周。于第8周末,各組大鼠收集血液、尿液及腎組織標本。然后檢測血糖(BG)、總膽固醇(TC)、甘油三酯(TG)、低密度脂蛋白膽固醇(LDL-C)、血肌酐(Scr)、尿素氮(BUN)、24小時尿白蛋白定量。酶聯(lián)免疫分析法(ELISA)測定各組大鼠血清vWF的濃度。腎組織進行蘇木素-伊紅(HE)染色在光鏡觀察腎臟形態(tài)學變化。用免疫組化的方法檢測各組大鼠腎臟vWF的表達,應用圖像分析系統(tǒng)進行分析。應用實時熒光定量PCR技術(shù)檢測腎組織vWF、PAI-1mRNA的表達情況。應用SPSS16.0統(tǒng)計學軟件處理實驗數(shù)據(jù),采用單因素方差分析、非參數(shù)檢驗實驗方法,以P0.05為差異有統(tǒng)計學意義。 結(jié)果: 18周后,B組大鼠BG、TC、TG、LDL-C、Scr、BUN、24小時尿白蛋白定量、腎臟肥大指數(shù)明顯高于A組(P0.01),應用丹紅注射液、二甲雙胍干預8周后,干預組大鼠TC、TG、LDL-C、Scr、BUN、24小時尿白蛋白定量、腎臟肥大指數(shù)較B組明顯減低(P0.05),D組BG較B組相比明顯降低(P0.01)。 2血清vWF:B組血清vWF的濃度較A組明顯升高(P0.01),而C組、D組較B組水平有所下降(P0.01),差異有統(tǒng)計學意義。 3形態(tài)學檢查:糖尿病大鼠出現(xiàn)病理改變,B組大鼠可見腎小球平均截面積明顯增大,系膜基質(zhì)輕度增多,部分腎小管上皮細胞萎縮,管腔擴張。C組、D組大鼠腎臟病理改變減輕。 4腎組織vWF蛋白的表達:免疫組化結(jié)果提示B組腎組織vWF蛋白表達明顯高于A組(P0.01);C組、D組大鼠腎臟vWF的表達較B組明顯減少(P0.01),但仍多于A組。 5實時熒光定量PCR結(jié)果提示:與A組相比,B組vWF、PAI-1mRNA的表達明顯增加(P0.01),而C組、D組vWF、PAI-1mRNA表達較B組有所減少(P0.05)。B組、C組、D組大鼠腎組織vWFmRNA相對表達量分別是A組的5.09倍、3.45倍、3.19倍。B組、C組、D組大鼠腎組織PAI-1mRNA相對表達量分別是A組的4.31倍、2.70倍、2.98倍。 結(jié)論: 1實驗性1型糖尿病大鼠血清vWF水平及腎臟vWF、PAI-1表達增加,提示vWF、PAI-1與糖尿病腎病的發(fā)生、發(fā)展關(guān)系密切。 2丹紅注射液、二甲雙胍通過調(diào)節(jié)血脂、降低尿白蛋白、降低vWF和PAI-1的表達,來發(fā)揮腎臟保護作用。
[Abstract]:Objective : Diabetic nephropathy ( DN ) is one of the most common and serious microvascular complications of diabetes mellitus .
Methods : Forty - eight male SD rats were randomly divided into two groups : normal control group ( group A ) and experimental group ( n = 38 ) . The rats were divided into three groups : diabetic group ( group B ) , total cholesterol ( TC ) , triglyceride ( TG ) , low density lipoprotein cholesterol ( LDL - C ) , serum creatinine ( TC ) , urea nitrogen ( BUN ) and urine albumin .
Results :
After 18 weeks , BG , TC , TG , LDL - C , BUN , 24h urine albumin and renal hypertrophy index in group B were significantly higher than those in group A ( P0.01 ) . After 8 weeks of intervention with Danhong injection and metformin , the levels of TC , TG , LDL - C , BUN , 24h urine albumin were significantly lower in the intervention group than in group B ( P0.05 ) , while those in group D were significantly lower than those in group B ( P0.01 ) .
There was a significant difference in the levels of the serum levels in the serum from the group A and the group D ( P0.01 ) , and the difference was statistically significant .
The pathological changes in diabetic rats showed that the mean cross - sectional area of glomerulus in group B was obviously increased , the mesangial matrix was slightly increased , partial renal tubular epithelial cells were atrophied , the lumen was dilated , and the pathological changes of renal pathology in group C and D were reduced .
The results showed that there was a significant difference in the levels of plasma protein in renal tissue in group B than in group A ( P0.01 ) .
In group C and D rats , the expression of VWF decreased significantly compared with group B ( P0.01 ) , but there were more than group A .
Results : Compared with group A , the expression of PAI - 1 mRNA in group B , group C and D was 5.09 times , 3.45 times and 3.19 times as compared with group A , and the relative expression of PAI - 1 mRNA in group B , group C and D was 4.31 times , 2.70 times and 2.98 times of group A , respectively .
Conclusion :
Results The levels of serum levels and the level of PAI - 1 in the serum of experimental type 1 diabetic rats were increased .
