【摘要】：目的:探討深圳地區(qū)IgA腎病患者的臨床病理特征。方法:回顧性分析692例經(jīng)腎活檢確診為IgA腎病患者的臨床病理資料。結(jié)果 :深圳地區(qū)的IgA腎病患者以青年居多。各年齡組之間舒張壓、24 h尿蛋白定量、鏡下血尿、白細(xì)胞、血膽固醇、血白蛋白、血IgA、血C_3,血糖水平差異無統(tǒng)計(jì)學(xué)意義(P0.05)。老年組的收縮壓、血肌酐、腎小球?yàn)V過率(eGFR)、血尿酸、血IgG與青、中年組比較存在差異(P0.05)。青少年組及老年組感染比率較中年組高(P0.05)。病理方面,不同年齡組在腎小管間質(zhì)積分、腎小動脈積分及總積分差異有統(tǒng)計(jì)學(xué)意義(P0.05),腎小球積分差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:深圳地區(qū)IgA腎病屬于常見的腎小球疾病之一。以青少年發(fā)病為主,老年IgA腎病患者腎小管間質(zhì)與血管的病變重于青中年組;感染在老年患者比較常見。
[Abstract]:Objective: to investigate the clinicopathological features of IgA nephropathy in Shenzhen area. Methods: the clinical and pathological data of 692 patients with IgA nephropathy diagnosed by renal biopsy were analyzed retrospectively. Results: the majority of patients with IgA nephropathy in Shenzhen area were young people. There was no significant difference in blood urine, white blood cell, serum cholesterol, serum albumin, serum IgA, blood C3 and blood glucose level between different age groups (P 0.05). The systolic blood pressure, serum creatinine, glomerular filtration rate (GFR), serum uric acid, serum IgG in the aged group were significantly different from those in the young and middle age group (P 0.05). The infection rate in the young group and the elderly group was higher than that in the middle age group (P 0.05). Pathologically, there were significant differences in tubulointerstitial integral, renal arteriole score and total integral between different age groups (P 0.05), but no significant difference in glomerular integral (P 0.05). Conclusion: IgA nephropathy is one of the common glomerular diseases in Shenzhen. The renal tubulointerstitial and vascular lesions were more serious in elderly patients with IgA nephropathy than in young and middle aged patients, and infection was more common in elderly patients.
