長(zhǎng)期血液透析患者生物型假體髖關(guān)節(jié)置換術(shù)的療效分析

【摘要】:目的分析生物型髖關(guān)節(jié)假體在長(zhǎng)期血液透析并發(fā)髖關(guān)節(jié)相關(guān)疾病患者行全髖關(guān)節(jié)置換(THA)的長(zhǎng)期穩(wěn)定性。方法回顧性分析2007-2016年重慶市人民醫(yī)院(三院院區(qū))收治的因慢性腎衰竭接受長(zhǎng)期血液透析并發(fā)髖關(guān)節(jié)相關(guān)疾病的患者7例(11個(gè)髖關(guān)節(jié)),觀察其髖臼及股骨柄假體松動(dòng)程度及術(shù)后并發(fā)癥的發(fā)生率。結(jié)果所有患者隨訪12~60個(gè)月(平均38個(gè)月)。D′Aubigne和Postel評(píng)分系統(tǒng)髖關(guān)節(jié)評(píng)分從術(shù)前平均7.7分恢復(fù)到術(shù)后14.5分。所有股骨柄假體均出現(xiàn)骨長(zhǎng)入。雖然1個(gè)股骨柄假體出現(xiàn)下沉,但隨訪2年后未出現(xiàn)繼續(xù)松動(dòng)。廣泛涂層生物型假體可在慢性腎功能不全患者THA后出現(xiàn)有效骨長(zhǎng)入。結(jié)論生物型髖關(guān)節(jié)假體置換是長(zhǎng)期血液透析并發(fā)髖關(guān)節(jié)相關(guān)疾病患者的有效治療方案。
[Abstract]:Objective to analyze the long-term stability of biological hip prosthesis with total hip replacement (THA) in hemodialysis patients with hip-related diseases. Methods from 2007 to 2016, 7 patients (11 hips) with chronic renal failure who received long-term hemodialysis complicated with hip joint related diseases were analyzed retrospectively in Chongqing people's Hospital (third Hospital). The acetabular and femoral pedicle prostheses were observed. Degree of loosening and incidence of postoperative complications. Results all the patients were followed up for 12 ~ 60 months (mean 38 months). The hip score of Aubigne and Postel system recovered from 7.7 before operation to 14.5 after operation. Bone growth was found in all femoral stalk prostheses. Although one femoral prosthesis sank, no further loosening occurred after 2 years follow-up. A wide-coated biological prosthesis can produce effective bone entry after tha in patients with chronic renal insufficiency. Conclusion Biologic hip prosthesis replacement is an effective treatment for long-term hemodialysis patients with hip-related diseases.
