【摘要】：目的探討老年患者醫(yī)院下尿路感染的易感因素與治療策略,為老年患者醫(yī)院下尿路感染的預(yù)防控制提供參考。方法回顧性分析2016年1月-12月醫(yī)院住院治療的196例老年患者臨床資料,調(diào)查患者下尿路感染率,分析感染病原菌分布及感染的相關(guān)因素,并比較感染患者與非感染患者治愈時(shí)間及癥狀緩解時(shí)間。結(jié)果 196例患者下尿路感染86例,感染率為43.88%,感染病原微生物以細(xì)菌74例為主,占86.05%;女性、患有惡性腫瘤及腎臟疾病、有尿道創(chuàng)傷性操作、有反復(fù)尿路感染病史、有糖尿病史、有低蛋白血癥是引起老年患者發(fā)生下尿路感染的危險(xiǎn)因素(P0.05);Logistic回歸分析顯示女性、有尿道創(chuàng)傷性操作、有反復(fù)尿路感染病史、有糖尿病史是引起老年患者發(fā)生醫(yī)院下尿路感染的獨(dú)立易感因素(P0.05);感染患者治愈時(shí)間及癥狀緩解時(shí)間分別為(11.19±3.20)d、(6.96±1.55)d,明顯長于非感染患者的(8.19±2.58)d、(4.20±1.06)d,比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);所有患者均在積極治療后治愈,治愈率為100.00%。結(jié)論女性、有尿道創(chuàng)傷性操作、有反復(fù)尿路感染病史、有糖尿病史是引起老年患者發(fā)生醫(yī)院下尿路感染的獨(dú)立易感因素,醫(yī)院下尿路感染老年患者治愈時(shí)間明顯延長,積極治療轉(zhuǎn)歸良好。
[Abstract]:Objective to explore the susceptible factors and treatment strategies of nosocomial urinary tract infection in elderly patients, and to provide reference for the prevention and control of nosocomial urinary tract infection in elderly patients. Methods the clinical data of 196 elderly patients hospitalized in hospital from January to December 2016 were retrospectively analyzed. The infection rate of lower urinary tract was investigated, and the distribution of pathogenic bacteria and the related factors of infection were analyzed. The time of cure and remission of symptoms were compared between infected patients and non-infected patients. Results there were 86 cases of lower urinary tract infection in 196 patients, the infection rate was 43.88, and 74 cases were bacteria (86.05%), the female had malignant tumor and kidney disease, traumatic urethra operation, history of repeated urinary tract infection, history of diabetes mellitus. Hypoproteinemia was a risk factor for lower urinary tract infection in elderly patients. Logistic regression analysis showed that women had traumatic urethral operation and had a history of recurrent urinary tract infection. The history of diabetes mellitus was an independent susceptible factor of nosocomial urinary tract infection in elderly patients (P 0.05), and the cure time and symptom remission time of infected patients were 6.96 鹵1.55 days, respectively, which were significantly longer than that of non-infected patients (4.20 鹵1.06 days), and the difference was statistically significant (P 0.05). All patients were cured after active treatment. The cure rate was 100.00000. Conclusion female patients with traumatic urethral operation, history of repeated urinary tract infection and diabetes mellitus are independent susceptible factors of nosocomial urinary tract infection. The cure time of elderly patients with nosocomial urinary tract infection is obviously prolonged. Positive treatment turned out to be good.
