【摘要】：為探討雷公藤多苷聯(lián)合激素治療成人腎病綜合征(NS)的臨床效果及對(duì)患者糖皮質(zhì)激素抵抗、熱休克蛋白90(HSP90)表達(dá)的影響,本研究將82例腎病綜合征患者按隨機(jī)數(shù)字表法分為對(duì)照組與觀察組兩組,每組41例,兩組均予以基礎(chǔ)對(duì)癥處理,對(duì)照組采用糖皮質(zhì)激素潑尼松治療,1 mg/kg,8周或尿蛋白陰轉(zhuǎn)2周后逐漸減量,維持20 mg/d,觀察組加用雷公藤多苷片治療,20 mg/次,3次/d,共治療24周。并將觀察組與對(duì)照組比較,結(jié)果發(fā)現(xiàn),觀察組治療總有效率高于對(duì)照組(p0.05),且治療后,兩組尿蛋白定量降低,總蛋白(TP)、血清白蛋白(ALB)均上升,與同組治療前對(duì)比差異有統(tǒng)計(jì)學(xué)意義(p0.05),觀察組尿蛋白定量降低幅度、TP及ALB上升幅度高于對(duì)照組(p0.05);且觀察組血肌酐(Cr)、尿素氮(BUN)、糖皮質(zhì)激素受體β(GRβ)、HSP90均降低,與同組治療前及對(duì)照組治療后對(duì)比差異有統(tǒng)計(jì)學(xué)意義(p0.05);同時(shí)觀察組不良反應(yīng)發(fā)生率略低于對(duì)照組,但未見統(tǒng)計(jì)學(xué)差異。對(duì)成人NS患者采用雷公藤多苷聯(lián)合糖皮質(zhì)激素治療較單純激素治療總有效率高,且可明顯改善患者腎功能及糖皮質(zhì)激素抵抗,下調(diào)HSP90表達(dá)。
[Abstract]:To investigate the clinical effect of tripterygium wilfordii polyglycoside combined with hormone in the treatment of adult nephrotic syndrome (NS) and its effect on glucocorticoid resistance and heat shock protein 90 (HSP90) expression. In this study, 82 patients with nephrotic syndrome were randomly divided into control group and observation group with 41 cases in each group. The control group was treated with glucocorticoid prednisone for 8 weeks or urine protein negative for 2 weeks. The control group was treated with tripterygium wilfordii polyglycosides for 20 mg/ d for 24 weeks. Compared with the control group, the total effective rate in the observation group was higher than that in the control group (P 0.05). Compared with the same group before treatment, the difference was statistically significant (p 0.05). The increase of TP and ALB in the observation group was higher than that in the control group, and the levels of creatinine, bun, glucocorticoid receptor 尾 -gr 尾 -HSP90 were decreased in the observation group, and the levels of serum creatinine, bun and glucocorticoid receptor 尾 -gr 尾 -HSP90 in the observation group were lower than those in the control group. The incidence of adverse reactions in the observation group was slightly lower than that in the control group, but there was no statistical difference between the observation group and the control group. The total effective rate of tripterygium wilfordii polyglycoside combined with glucocorticoid was higher than that of single hormone therapy, and the renal function and glucocorticoid resistance of the patients were significantly improved, and the expression of HSP90 was down-regulated.
