【摘要】：腎上皮樣血管平滑肌脂肪瘤(renal epithelioid angiomyolipoma,REAML)是一種臨床極其罕見的腎臟實性腫瘤,具有惡變潛能的特性,屬于血管周圍上皮樣細(xì)胞瘤腫瘤家族(PEComa),其在組織學(xué)上主要由上皮樣細(xì)胞組成。在惡性REAML組織中可以發(fā)現(xiàn)P53基因突變,可能在REAML的惡性轉(zhuǎn)化中起了關(guān)鍵作用。由于臨床醫(yī)師對其認(rèn)識尚淺,臨床表現(xiàn)無特異性,常規(guī)影像學(xué)檢查不能明確診斷,因此臨床診斷難度比較大,容易導(dǎo)致誤診為腎細(xì)胞癌,病理結(jié)果及免疫組化表型結(jié)果是診斷和鑒別診斷的有效依據(jù),首選黑色素相關(guān)蛋白HMB-45和Melan-A。REAML目前尚無有效治療方法,但臨床上仍首選外科手術(shù)治療,術(shù)后仍有可能出現(xiàn)復(fù)發(fā)及轉(zhuǎn)移,則預(yù)后較差,生存期較短,死亡率較高,需長期密切隨訪觀察。對于不能手術(shù)治療的患者,可試用化療、m TOR抑制劑或伊馬替尼等新型靶向藥物治療,但臨床療效仍有待進一步驗證。
[Abstract]:Renal epithelioid angiomyolipoma (REAMLR) is a rare clinical solid renal tumor with malignant potential. It belongs to the tumor family of perivascular epithelioid cell tumor (PEComaI), which is mainly composed of epithelioid cells in histology. P53 gene mutation can be found in malignant REAML tissues, which may play a key role in the malignant transformation of REAML. Because the clinicians know little about it, the clinical manifestation is not specific, and the routine imaging examination can not make a definite diagnosis, so the clinical diagnosis is difficult, and it is easy to be misdiagnosed as renal cell carcinoma (RCC). Pathological results and immunohistochemical phenotypic results are the effective basis for diagnosis and differential diagnosis. There is no effective treatment method for melanin associated protein HMB-45 and Melan-A.REAML, but surgery is still the first choice in clinic. Recurrence and metastasis may still occur after the operation, the prognosis is poor, the survival time is shorter, the mortality is higher, and it is necessary to follow up closely for a long time. New targeted drugs such as chemotherapeutic TOR inhibitor or imatinib can be used in patients who can not be treated surgically, but the clinical efficacy needs to be further verified.
