HIF-1α及GLUT-1在前列腺癌中的表達(dá)及意義

【摘要】:背景 前列腺癌(prostate cancer, PCa)是男性泌尿生殖系統(tǒng)常見腫瘤之一。目前對(duì)其治療主要為手術(shù)及內(nèi)分泌治療。由于患者多為老年病人且早期臨床癥狀不明顯。因此,多數(shù)就診時(shí)腫瘤已存在周邊侵襲及遠(yuǎn)處轉(zhuǎn)移的情況,進(jìn)而失去根治性手術(shù)機(jī)會(huì);內(nèi)分泌治療雖能夠緩解病情,但無法從根本上治愈。因此,尋找特異性較高的列腺癌早期診斷方法已成為治療前列腺癌的關(guān)鍵因素,同時(shí)也可能為前列腺癌的靶向治療及判斷預(yù)后提供新的參考。 目的 本研究以前列腺腺癌患者的癌組織及正常前列腺增生組織為研究對(duì)象,采用免疫組化方法檢測前列腺腺癌組織中缺氧誘導(dǎo)因子HIF-1α及葡萄糖轉(zhuǎn)運(yùn)蛋白GLUT-1的表達(dá)水平,分析其表達(dá)水平與前列腺腺癌臨床病理特征的關(guān)系;同時(shí)探索前列腺腺癌中缺氧誘導(dǎo)因子HIF-1α及葡萄糖轉(zhuǎn)運(yùn)蛋白GLUT-1的表達(dá)相關(guān)性,初步探討它們在前列腺腺癌發(fā)生、發(fā)展中的作用及其臨床應(yīng)用價(jià)值。 方法 收集從2012年2月到2013年8月于新鄉(xiāng)醫(yī)學(xué)院第一附屬醫(yī)院泌尿外科手術(shù)切除前列腺腺癌標(biāo)本46例,其中12例行恥骨后前列腺根治術(shù);34例行經(jīng)尿道前列腺電切術(shù),標(biāo)本均經(jīng)病理學(xué)確診為前列腺腺癌,術(shù)前均未經(jīng)過化療和放療;另取40例前列腺增生組作為對(duì)照。組織采用多聚甲醛固定、脫水、石蠟包埋、3um連續(xù)切片,并烘干、脫蠟、水化、進(jìn)行免疫組化(DAB法)染色,檢測前列腺腺癌組織及前列腺增生組織中HIF-la及GLUT-1蛋白的表達(dá)情況,探討HIF-1α、GLUT-1的表達(dá)水平與前列腺腺癌病理分級(jí)及臨床分期的關(guān)系以及HIF-1α、GLUT-1蛋白在前列腺腺癌組織中的表達(dá)相關(guān)性。每個(gè)切片分別隨機(jī)選取不重疊的5個(gè)高倍鏡視野,分別由兩名不知道切片分組的觀察者,觀察并計(jì)數(shù)陽性染色細(xì)胞數(shù)。采用AB值法(半定量法)進(jìn)行相關(guān)分析。 結(jié)果 免疫組化DAB染色法結(jié)果顯示,HIF-1α和GLUT-1蛋白在前列腺腺癌組織中的陽性表達(dá)率分別為60.9%和67.4%;前列腺增生組織中其陽性表達(dá)率分別為12.5%和17.5%,兩者相比,HIF-1α和GLUT-1在前列腺腺癌中的表達(dá)顯著增高,其差異具有統(tǒng)計(jì)學(xué)意義(P0.05); HIF-1α和GULT-1在前列腺癌中的表達(dá)與患者的臨床病理分級(jí)及分期關(guān)系如下:在病理分級(jí)低分化、臨床分期晚及伴有淋巴結(jié)及遠(yuǎn)處轉(zhuǎn)移的標(biāo)本中HIF-1α和GULT-1的陽性表達(dá)率明顯高于病理高分化、臨床分期早及無腫瘤轉(zhuǎn)移者,其差異具有統(tǒng)計(jì)學(xué)意義(P0.05); HIF-1α和GLUT-1蛋白在前列腺腺癌組織中的陽性表達(dá)呈顯著正相關(guān)關(guān)系(P0.05)。 結(jié)論1.HIF-1α及GLUT-1在前列腺腺癌組織中呈正相關(guān)性高表達(dá),且與腫瘤臨床病理特征密相關(guān),推測其可能在前列腺腺癌的發(fā)生及發(fā)展過程中起到至關(guān)重要的作用。 2. HIF-1α與GLUT-1可成為前列腺腺癌的生物行為學(xué)檢測指標(biāo)。
[Abstract]:Background Prostate cancer (PCA) is one of the most common tumors in male genitourinary system. At present, the main treatment is surgery and endocrine therapy. Because the patients are mostly elderly patients and early clinical symptoms are not obvious. Therefore, most of the tumors have been invasion and distant metastasis, and then lost the opportunity of radical surgery. Endocrine therapy can alleviate the disease, but can not be fundamentally cured. Therefore, the search for a highly specific method for early diagnosis of prostate cancer has become a key factor in the treatment of prostate cancer, and may also provide a new reference for the targeted treatment of prostate cancer and the judgment of prognosis. Purpose In this study, the expression of hypoxia inducible factor (HIF-1 偽) and glucose transporter (GLUT-1) in prostate adenocarcinoma tissues and normal prostatic hyperplasia tissues were detected by immunohistochemical method. To analyze the relationship between the expression level and clinicopathological features of prostate adenocarcinoma, and to explore the relationship between the expression of hypoxia inducible factor HIF-1 偽 and glucose transporter GLUT-1 in prostate adenocarcinoma, and to explore their occurrence in prostate adenocarcinoma. The role of development and its clinical application value. Method From February 2012 to August 2013, 46 cases of prostatic adenocarcinoma were treated by urological surgery in the first affiliated Hospital of Xinxiang Medical College. Among them, 12 cases were treated with retropubic radical prostatectomy and 34 cases with transurethral resection of prostate. All the specimens were pathologically diagnosed as prostatic adenocarcinoma without chemotherapy and radiotherapy before operation, and 40 cases of benign prostatic hyperplasia were taken as control group. The tissue was fixed with paraformaldehyde, dehydrated, paraffin embedded in 3um serial section, and dried, dewaxed, hydrated, and stained by immunohistochemical staining. The expression of HIF-la and GLUT-1 in prostate adenocarcinoma and prostatic hyperplasia was detected. To investigate the relationship between the expression of HIF-1 偽 -GLUT-1 and the pathological grade and clinical stage of prostate adenocarcinoma, and the correlation between the expression of HIF-1 偽 -GLUT-1 protein in prostate adenocarcinoma tissue. Each section randomly selected 5 high-power field of vision without overlapping. Two observers who did not know how to divide the sections were selected to observe and count the number of positive staining cells. AB value method (semi-quantitative method) was used for correlation analysis. Result The positive expression rates of HIF-1 偽 and GLUT-1 protein were 60.9% and 67.4% in prostate adenocarcinoma and 12.5% and 17.5% in benign prostatic hyperplasia, respectively. The positive expression rates of HIF-1 偽 and GLUT-1 in prostate gland were higher than that of HIF-1 偽 and GLUT-1 in prostate gland. There was a significant increase in expression in cancer, The relationship between the expression of HIF-1 偽 and GULT-1 in prostate cancer and the clinicopathological grades and stages of prostate cancer was as follows: low differentiation in pathological grade, low differentiation in pathological grade, The positive rates of HIF-1 偽 and GULT-1 in patients with late clinical stage and lymph node and distant metastasis were significantly higher than those with pathological differentiation. The positive expression of HIF-1 偽 and GLUT-1 protein in prostate adenocarcinoma was positively correlated with that of P0.05. Conclusion the expression of 1.HIF-1 偽 and GLUT-1 in prostate adenocarcinoma is positively correlated with the clinicopathological features of prostate adenocarcinoma. It is suggested that the expression of 1.HIF-1 偽 and GLUT-1 may play an important role in the occurrence and development of prostate adenocarcinoma. 2. HIF-1 偽 and GLUT-1 can be used as biomedical indicators for prostate adenocarcinoma.
