【摘要】：目的探討經(jīng)后腹腔自制單孔通道腹腔鏡腎癌根治術(shù)治療T1期腎癌的臨床應(yīng)用價(jià)值。方法 T1期腎癌患者80例,其中應(yīng)用自制通道行后腹腔單孔腹腔鏡腎癌根治術(shù)40例,傳統(tǒng)后腹腔鏡腎癌根治術(shù)40例;回顧性分析和比較兩組患者在手術(shù)時(shí)間、術(shù)中出血量、術(shù)后住院時(shí)間、術(shù)后疼痛評(píng)分、切口美容滿意度、手術(shù)并發(fā)癥及生存率上的差異。結(jié)果兩組手術(shù)均順利完成,無中轉(zhuǎn)開放或傳統(tǒng)腹腔鏡者。經(jīng)后腹腔自制單孔通道腹腔鏡組與傳統(tǒng)后腹腔鏡組的術(shù)后疼痛評(píng)分〔(3.63±0.87)分vs(4.52±1.24)分〕、切口美容滿意度〔(7.95±0.60)分vs(6.93±1.21)分〕差異顯著(P0.05)。術(shù)后隨訪時(shí)間3~40個(gè)月,兩組患者均未出現(xiàn)腫瘤復(fù)發(fā)或轉(zhuǎn)移。結(jié)論經(jīng)后腹腔自制單孔通道腹腔鏡腎癌根治術(shù)是安全可行的。與傳統(tǒng)后腹腔鏡腎癌根治術(shù)相比,術(shù)后疼痛更輕、切口美容滿意度更高。腫瘤治療的遠(yuǎn)期療效仍需進(jìn)一步長期隨訪予以驗(yàn)證。
[Abstract]:Objective to evaluate the clinical value of retroperitoneal single channel laparoscopic radical nephrectomy for T1 stage renal cell carcinoma. Methods A total of 80 patients with T1 stage renal cell carcinoma were treated with self-made channels, including 40 patients with retroperitoneal laparoscopic radical nephrectomy and 40 patients with traditional retroperitoneal laparoscopic renal cell carcinoma, the time of operation and the amount of intraoperative bleeding were retrospectively analyzed and compared between the two groups. Differences in postoperative hospital stay, postoperative pain score, incision beauty satisfaction, surgical complications and survival rate. Results the operation was completed successfully in both groups, and there was no transition to open operation or traditional laparoscopy. The postoperative pain score was 3.63 鹵0.87 (vs(4.52 鹵1.24) and the incision beauty satisfaction was 7.95 鹵0.60 (vs(6.93 鹵1.21). The follow-up time was 3 ~ 40 months. No tumor recurrence or metastasis was found in the two groups. Conclusion Laparoscopic radical resection of renal cell carcinoma with self-made self-made peritoneal cavity is safe and feasible. Compared with traditional retroperitoneal laparoscopic radical nephrectomy, postoperative pain is lighter and incision beauty satisfaction is higher. The long-term effect of tumor treatment needs further long-term follow-up to verify.
