【摘要】：目的比較輸尿管軟鏡碎石術(shù)(F-URS)與微創(chuàng)經(jīng)皮腎鏡碎石術(shù)(M-PCNL)治療孤立腎結(jié)石的效果。方法 53例孤立腎結(jié)石患者,其中22例接受F-URS治療(F-URS組)、31例接受M-PCNL治療(M-PCNL組),觀察兩組手術(shù)時間、術(shù)后血紅蛋白下降值、術(shù)后腸道功能恢復(fù)時間、住院時間、結(jié)石清除率及并發(fā)癥發(fā)生情況。結(jié)果 FURS組、M-PCNL組手術(shù)時間分別為(78.2±33.4)、(62.3±23.2)min,術(shù)后血紅蛋白下降值分別為(1.6±0.7)、(4.3±2.3)g/L,術(shù)后腸道功能恢復(fù)時間分別為(27.6±15.1)、(36.4±11.3)h,住院時間分別為(4.1±1.1)、(7.6±1.3)d。兩組術(shù)后血紅蛋白下降值、住院時間比較,P均0.01。F-URS組、M-PCNL組一期結(jié)石清除率分別為68.2%、93.5%,總清除率分別為86.4%、96.8%,下盞結(jié)石一期結(jié)石清除率分別為28.6%、90.9%,下盞結(jié)石總清除率分別為57.1%、90.9%,直徑≤20 mm結(jié)石總清除率分別為92.3%、100.0%,直徑20 mm結(jié)石總清除率分別為75.0%、90.0%。與F-URS組相比,M-PCNL組一期結(jié)石清除率、下盞結(jié)石一期結(jié)石清除率高(P均0.05)。F-URS組、M-PCNL組大出血分別為0、3例,高熱分別為3、2例,胸膜損傷分別為0、1例。結(jié)論與M-PCNL相比,F-URS治療孤立腎結(jié)石一期結(jié)石清除率低,術(shù)后血紅蛋白下降少、住院時間短,但兩者結(jié)石總清除率、并發(fā)癥發(fā)生情況無差異。
