【摘要】：目的比較表觀擴(kuò)散系數(shù)(ADC)值和標(biāo)化ADC值在診斷前列腺癌惡性程度中的價值。方法回顧性分析經(jīng)病理證實(shí)的34例前列腺癌的彌散加權(quán)成像(DWI)掃描結(jié)果。測量患者49個癌灶及外周帶正常區(qū)域的ADC值,計算癌灶標(biāo)化ADC值。根據(jù)病理Gleason評分,49個癌灶分為:≤6分(低危組)、7分(中危組)、≥8分(高危組)共3組。對各組ADC值之間和標(biāo)化ADC值之間的差別及ADC值和標(biāo)化ADC值與Gleason評分之間的相關(guān)性進(jìn)行統(tǒng)計學(xué)分析。結(jié)果前列腺癌ADC值及標(biāo)化ADC值3組整體差異均有統(tǒng)計學(xué)意義(P0.01),但兩兩比較ADC值在中危組與高危組之間差異無統(tǒng)計學(xué)意義(P0.05),而標(biāo)化ADC值在各組間差異均有統(tǒng)計學(xué)意義(P0.05)。ADC值(r=-0.546,P=0.000)和標(biāo)化ADC值(r=-0.575,P=0.000)與Gleason評分均呈負(fù)相關(guān)。在比較前列腺癌各組的ROC曲線下面積時,標(biāo)化ADC值與ADC值之間的差異無統(tǒng)計學(xué)意義(P0.05),但當(dāng)特異性調(diào)整至100%時,標(biāo)化ADC值在區(qū)別低危組與中危組、低危組與高危組時較ADC值具有更高的靈敏性(47.5%vs.5.6%、78.2%vs.50.9%)。結(jié)論 ADC值和標(biāo)化ADC值與Gleason評分均呈負(fù)相關(guān),但標(biāo)化ADC值在診斷前列腺癌惡性程度方面可能具有更高的效能。
[Abstract]:Objective to compare the value of apparent diffusion coefficient (ADCC) and standardized ADC in the diagnosis of malignant degree of prostate cancer.
