【摘要】：目的:研究年齡對經(jīng)尿道等離子前列腺剜除術(shù)(PKEP)治療前列腺增生癥(BPH)的影響。評價經(jīng)尿道等離子前列腺剜除術(shù)治療不同年齡患者療效及安全性。方法:收集山西醫(yī)科大學(xué)第一醫(yī)院2014年6月到2016年5月之間行經(jīng)尿道等離子前列腺剜除術(shù)的前列腺增生癥住院患者,將患者分為兩組:非高齡組,年齡60~75歲,30例;高齡組,年齡75歲,30例。收集患者術(shù)前1周、術(shù)后2周及術(shù)后6月的國際前列腺癥狀評分(IPSS)、最大尿流率(Qmax)、殘余尿量(PVR)和生活質(zhì)量評分(QOL)及兩組手術(shù)時間、術(shù)中出血量、切除組織重量、術(shù)后住院時間、術(shù)后置管時間,手術(shù)參照標(biāo)準(zhǔn)手術(shù)步驟進(jìn)行。采用SPSS 19.0統(tǒng)計學(xué)軟件對數(shù)據(jù)進(jìn)行處理,計數(shù)資料以率表示,采用X2檢驗;計量資料如服從正態(tài)分布,以(—X±S)表示,比較采用t檢驗;如不服從正態(tài)分布,則以(M±QR)表示,采用秩和檢驗。以P0.05認(rèn)為差異有統(tǒng)計學(xué)意義。結(jié)果:1.高齡組(年齡在大于75歲)患者尿潴留(P=0.038 P0.05)的發(fā)生率顯著增高,二者差距有顯著性差異;2.非高齡組與高齡組的術(shù)后住院時間分別是5.1±1.4和6.2±2.1 P=0.02,二者有顯著性差異(P0.05);3.非高齡組與高齡組的手術(shù)前和手術(shù)后IPSS、Qmax、PVR、Qol在統(tǒng)計學(xué)上有顯著性差異,具有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:經(jīng)尿道等離子前列腺剜除術(shù)治療不同年齡患者無明顯差別,療效和安全性確切�？傊�,經(jīng)尿道等離子前列腺剜除術(shù)治療老年前列腺增生癥患者是一種安全有效的手術(shù)方式。
[Abstract]:Objective: to study the effect of age on the treatment of benign prostatic hyperplasia (BPH) by transurethral plasma enucleation of prostate (PKEPs).To evaluate the efficacy and safety of transurethral plasma enucleation of the prostate in patients of different ages.Methods: the inpatients with benign prostatic hyperplasia who underwent transurethral plasma enucleation of the prostate between June 2014 and May 2016 in the first Hospital of Shanxi Medical University were collected and divided into two groups: the non-elderly group (n = 30) and the elderly group (n = 30).30 cases were aged 75 years.The international prostatic symptom score (IPSS), maximal urinary flow rate (Qmaxus), residual urine volume (PVR) and quality of life score (QOLL) were collected 1 week before operation, 2 weeks after operation and 6 months after operation. The operative time, intraoperative bleeding volume, weight of resected tissue, and postoperative hospitalization time were collected.The time of catheterization after operation and the standard procedure of operation were carried out.SPSS 19.0 statistical software was used to process the data, the counting data was expressed as rate and X2 test was used; the measurement data, such as normal distribution, were expressed as -X 鹵S, and the t test was used to compare the data, and if the normal distribution was not taken, the data were expressed as M 鹵QR.Rank sum test was used.P0.05 thought the difference was statistically significant.The result is 1: 1.The incidence of urinary retention in elderly patients (aged more than 75 years) was significantly higher than that in patients with urinary retention (P < 75), and there was a significant difference between the two groups (P < 0.05).The postoperative hospitalization time was 5.1 鹵1.4 and 6.2 鹵2.1 P0. 02 in the non-elderly group and the elderly group, respectively. There was a significant difference between the two groups.Before and after operation, there was statistically significant difference between the non-elderly group and the elder group in IPSS Q maxPVRN Qol, which had statistical significance (P 0.05).Conclusion: transurethral plasma enucleation of the prostate has no significant difference in the treatment of patients of different ages.In conclusion, transurethral plasma enucleation of the prostate is a safe and effective procedure for elderly patients with benign prostatic hyperplasia.
