【摘要】：目的:觀察經(jīng)腹膜外途徑腹腔鏡下治療雙側(cè)精索靜脈曲張高位結(jié)扎的療效。方法:回顧性分析48例經(jīng)腹膜外途經(jīng)腹腔鏡下精索靜脈高位結(jié)扎術及32例經(jīng)腹腔途經(jīng)腹腔鏡下精索靜脈高位結(jié)扎術患者。比較兩組患者術前、術后l、3、6個月的精液質(zhì)量變化情況,兩組患者術中出血量、手術時間、術后胃腸功能恢復時間、術后并發(fā)癥(皮下血腫、陰囊及皮下氣腫、精索靜脈曲張復發(fā))以及配偶妊娠率等情況。結(jié)果:經(jīng)腹膜外組和經(jīng)腹腔組術后精液質(zhì)量參數(shù)均較術前有明顯改善(P0.05),但是術后1、3、6個月的精液質(zhì)量參數(shù)比較無統(tǒng)計學差異(P0.05);兩組術中出血量、手術時間[(38.27±9.23)min vs(43.46±11.72)min],無明顯差異(P0.05),而胃腸道恢復時間經(jīng)腹膜外途徑組[(1.27±0.26)d]明顯短于經(jīng)腹腔途徑組[(2.43±0.41)d,P0.05]。術后隨訪6~18個月,兩組皮下血腫、陰囊及皮下氣腫、復發(fā)率及配偶妊娠率無統(tǒng)計學差異(P0.05)。結(jié)論:經(jīng)腹膜外途徑腹腔鏡下精索靜脈高位結(jié)扎術具有恢復快、安全有效等優(yōu)點,具有一定臨床推廣價值。
[Abstract]:Objective: to observe the effect of laparoscopic ligation of bilateral varicocele.Methods: 48 cases of high ligation of spermatic vein via laparoscope and 32 cases of high ligation of spermatic vein were analyzed retrospectively.The changes of semen quality were compared between the two groups before and after operation. Blood loss, operation time, postoperative gastrointestinal function recovery time, postoperative complications (subcutaneous hematoma, scrotal emphysema and subcutaneous emphysema) were compared between the two groups.Recurrent varicocele) and spouse pregnancy rate.Results: the semen quality parameters in the extraperitoneal group and the transperitoneal group were significantly improved compared with those before the operation, but there was no significant difference in the semen quality parameters between the two groups at 1 and 6 months after operation (P 0.05).The time of operation [38.27 鹵9.23)min vs(43.46 鹵11.72)min] had no significant difference (P 0.05), but the recovery time of gastrointestinal tract was significantly shorter in the extraperitoneal approach group [1.27 鹵0.26 days] than that in the peritoneal approach group [2.43 鹵0.41 days (P0.05)].Follow-up for 6 ~ 18 months showed that there was no significant difference in subcutaneous hematoma, scrotal and subcutaneous emphysema, recurrence rate and spouse pregnancy rate between the two groups (P 0.05).Conclusion: laparoscopic high ligation of spermatic vein via extraperitoneal approach has the advantages of quick recovery, safety and effectiveness, and has certain clinical value.
