【摘要】：目的利用重慶市某高校附屬醫(yī)院住院費用數(shù)據(jù),比較腹腔鏡前列腺根治術(laparoscopic radical prostatectomy,LRP)和恥骨后前列腺根治術(radical retropubic prostatectomy,RRP)住院費用的差異,探討導致兩者住院費用差異的主要原因。方法回顧分析該院2013年1月~2014年12月所有病理診斷為前列腺癌并行前列腺根治術(radical prostatectomy,RP)的83例出院患者的病歷資料(LRP 54例,RRP 29例)。摘錄病案首頁的社會人口學信息、住院診療信息和住院費用信息,住院費用信息包含藥費、檢查費、治療費、手術室費用、外科材料費和其他費用,比較LRP和RRP住院費用的差異。結果在年齡、住院時間、Gleason評分、臨床分期以及合并癥方面2組無顯著性差異(P0.05)。LRP住院總費用高于RRP[LRP(55 695±11 130)元,RRP(45 955±23 011)元,t=2.148,P=0.039],2組住院總費用的主要差異在治療費(中位數(shù)LRP 5862元,RRP4587元,Z=-2.511,P=0.011),手術室費用(中位數(shù)LRP 9620元,RRP 9265元,Z=-2.197,P=0.028)以及外科材料費(中位數(shù)LRP 24 911元,RRP 16 692元,Z=-3.009,P=0.003)。2組檢查費[(LRP(8521±2179)元,RRP(8256±3288)元,t=0.390,P=0.669]和藥費(中位數(shù)LRP 10 274元,RRP 10 182元,Z=-0.096,P=0.924)無顯著性差異(P0.05)。結論在臨床教學醫(yī)院,LRP的住院總費用高于RRP,其主要原因是LRP較高的手術室費用和外科材料費。
[Abstract]:Objective to compare the difference of hospitalization cost between laparoscopic radical prostatectomy (LRP) and retropubic radical prostatectomy (RRP) in a university affiliated hospital in Chongqing, and to explore the main reasons leading to the difference in hospitalization cost.Methods from January 2013 to December 2014, the medical records of 83 discharged patients with prostate cancer and radical prostatectomy were retrospectively analyzed.
