【摘要】：[目的]研制癌癥患者生命質(zhì)量測定量表體系之前列腺癌量表(The Quality of Life Instrument for Cancer Patients-prostate cancer, QLICP-PR)以及歐洲癌癥研究和治療組織的EORTC QLQ-PR55 (Quality of Life Questionnaire: EORTC QLQ-C30; QLQ-PR25)中文版量表,分別考評并比較兩個量表。[方法]依據(jù)共性模塊與特異模塊相結(jié)合的研究思想,通過大量回顧國內(nèi)外相關(guān)文獻(xiàn)資料,結(jié)合國內(nèi)文化特點,以開發(fā)量表的程序化決策方式研制QLICP-PR量表,主要包括：訪談、小組集中討論、統(tǒng)計分析結(jié)合臨床實際等。同時通過量表漢化的一系列包括翻譯、回譯、文化調(diào)試等步驟研制QLQ-PR25中文版,再同已有的QLQ-C30中文版結(jié)合形成QLQ-PR55中文版量表。采用兩個量表在入院時對115例前列腺癌患者進行第一次測試,其中,94例患者于入院2-3天進行第二次量表測試；99例患者于出院時進行第三次量表測試,通過最終的調(diào)查數(shù)據(jù)來考評兩個量表的測量學(xué)特性(信度、效度、反應(yīng)度),同時對兩個量表進行比較。在處理數(shù)據(jù)分析時采用的統(tǒng)計學(xué)方法有：統(tǒng)計描述、配對t檢驗、相關(guān)性分析、探索性因子分析等。[結(jié)果]1.研制出了含21個條目的前列腺癌特異模塊,與已有含32個條目的共性模塊QLICP-GM結(jié)合形成前列腺癌患者生命質(zhì)量量表QLICP-PR;同時,還研制出了EORTC QLQ-PR55中文版量表。2.對QLICP-PR量表進行考評,結(jié)果顯示該量表的重測信度為0.920,克朗巴赫系數(shù)為0.884,分半信度為0.837;概化理論研究結(jié)果顯示心理功能領(lǐng)域(9條)、社會功能領(lǐng)域(8條)、共性癥狀和副作用領(lǐng)域(7條)、特異模塊(21條)設(shè)置條目數(shù)適宜,而軀體功能領(lǐng)域(8條)應(yīng)適當(dāng)增加條目。相關(guān)分析和因子分析顯示QLICP-PR量表的結(jié)構(gòu)效度尚可,采用QLQ-PR55中文版量表作為效標(biāo)分析可知,QLICP-PR量表具有較好的效標(biāo)效度。除性功能問題外,其余領(lǐng)域治療前后得分的配對t檢驗均有統(tǒng)計學(xué)意義(P0.05),該量表基本能反映出患者治療前后生命質(zhì)量的變化情況。3.對QLQ-PR55中文版量表進行考評,并將QLICP-PR量表與QLQ-PR55中文版量表進行比較：從內(nèi)容上看,兩量表均能反映生命質(zhì)量測量的基本內(nèi)容,但存在異同點；從測量學(xué)特性上看,兩量表的信度、反應(yīng)度均較好,效度尚可,其中信度、結(jié)構(gòu)效度、反應(yīng)度接近,且均具有較好的內(nèi)容效度。[結(jié)論]前列腺癌患者生命質(zhì)量量表QLICP-PR有較好的信度、反應(yīng)度,效度尚可,可用于中國前列腺癌患者生命質(zhì)量的測評。QLICP-PR量表與QLQ-PR55中文版量表比較,內(nèi)容上存在異同點；測量學(xué)特征上兩量表類似。
[Abstract]:[objective] to develop the Quality of Life Instrument for Cancer Patients-prostate cancer (QLICP-PRA) and EORTC QLQ-PR55 quality of Life questionnaire naive: EORTC QLQ-C30; QLQ-PR25 of the European Organization for Research and treatment of Cancer. [methods] according to the research thought of the combination of common module and specific module, through reviewing a large number of domestic and foreign relevant literature materials, combined with the characteristics of domestic culture, The QLICP-PR scale is developed by developing the program decision of the scale, including interview, group discussion, statistical analysis combined with clinical practice, etc. At the same time, a series of Chinese version of the scale includes translation, backtranslation, etc. The Chinese version of QLQ-PR25 was developed and combined with the existing Chinese version of QLQ-C30 to form the Chinese version of QLQ-PR55. Two scales were used to test 115 patients with prostate cancer for the first time on admission. Among them, 94 patients were tested by the second scale on 2-3 days after admission. 99 patients were tested with the third scale when they were discharged from hospital. The measuring characteristics (reliability, validity) of the two scales were evaluated by the final survey data. The statistical methods used in processing data analysis are: statistical description, paired t test, correlation analysis. Exploratory factor analysis and so on. [results] 1. A 21 item prostate cancer specific module was developed, which was combined with QLICP-GM, a common module with 32 items, to form QLICP-PR1. The Chinese version of EORTC QLQ-PR55 is also developed. 2. The QLICP-PR scale is evaluated. The results showed that the test-retest reliability of the scale was 0.920, the Cronbach coefficient was 0.884, and the split-half reliability was 0.837. The results of the generalization theory showed that there were 9 cases in psychological function, 8 in social function, 7 in common symptoms and side effects. The number of items set is appropriate, Correlation analysis and factor analysis showed that the structure validity of QLICP-PR scale was fairly good. The Chinese version of QLQ-PR55 scale was used to analyze the validity of QLICP-PR. Except for sexual function problems, QLICP-PR scale had good validity. The paired t test of scores in other fields before and after treatment had statistical significance (P 0.05). The scale could basically reflect the changes of quality of life of patients before and after treatment. 3. The Chinese version of QLQ-PR55 was evaluated. Comparing the QLICP-PR scale with the Chinese version of QLQ-PR55, the two scales can reflect the basic content of the quality of life measurement, but there are similarities and differences, from the measurement characteristics, the reliability and response of the two scales are good, and the validity is good. Among them, reliability, structure validity, response degree were close, and all had good content validity. [conclusion] QLICP-PR has good reliability, response and validity in prostate cancer patients. The QLICP-PR scale can be used to evaluate the quality of life of prostate cancer patients in China. Compared with the Chinese version of QLQ-PR55, there are similarities and differences in the content of QLICP-PR and the two scales are similar in measuring characteristics.
