【摘要】：目的對(duì)比分析達(dá)芬奇內(nèi)窺鏡輔助前列腺根治術(shù)與腹腔鏡根治術(shù)術(shù)后醫(yī)院感染情況。方法選取醫(yī)院2015年6月-2016年6月行前列腺根治術(shù)患者86例為試驗(yàn)組,采用達(dá)芬奇內(nèi)窺鏡輔助前列腺根治術(shù)治療;2014年6月-2015年5月在醫(yī)院行腹腔鏡下前列腺根治術(shù)患者98例為對(duì)照組,比較兩組術(shù)后醫(yī)院感染發(fā)生情況。結(jié)果兩組在術(shù)后留置尿管時(shí)間上并無(wú)明顯差異,但試驗(yàn)組手術(shù)時(shí)間、術(shù)中出血量、術(shù)后住院時(shí)間均明顯少于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);試驗(yàn)組患者發(fā)生醫(yī)院感染7例,感染率為8.13%,對(duì)照組感染18例感染率為18.36%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);試驗(yàn)組感染患者共培養(yǎng)出病原菌18株,對(duì)照組檢出29株,均以革蘭陰性菌為主,分別占72.22%、75.86%。結(jié)論達(dá)芬奇內(nèi)窺鏡輔助前列腺根治術(shù)相比腹腔鏡根治術(shù),發(fā)生術(shù)后醫(yī)院感染概率更低,具有更好的臨床應(yīng)用價(jià)值。
[Abstract]:Objective to compare and analyze the nosocomial infection after radical prostatectomy with Leonardo da Vinci endoscope and laparoscopic radical prostatectomy. Methods 86 patients undergoing prostatectomy from June 2015 to June 2016 in our hospital were selected as the experimental group. Da Vinci endoscope was used to assist radical prostatectomy and 98 patients underwent laparoscopic prostatectomy in hospital from June 2014 to May 2015 as control group. Results there was no significant difference in the time of indwelling urethral catheter between the two groups, but the time of operation, the amount of intraoperative bleeding and the time of hospitalization after operation in the experimental group were significantly less than those in the control group. The nosocomial infection occurred in 7 patients in the test group, the infection rate was 8.13 and the infection rate in the control group was 18.366.There was a significant difference between the two groups. In the control group, 29 strains of Gram-negative bacteria were detected, accounting for 72.22% and 75.86% respectively. Conclusion compared with laparoscopic radical prostatectomy, Leonardo da Vinci endoscope assisted prostatectomy has a lower probability of postoperative nosocomial infection, and has better clinical application value.
