Gates法計(jì)算的GFR在單側(cè)腎積水患者腎小球?yàn)V過功能評(píng)價(jià)中的應(yīng)用

【摘要】:目的探討Gates法計(jì)算的腎小球?yàn)V過率(GFR)對(duì)單側(cè)腎積水患者腎小球?yàn)V過功能評(píng)估的價(jià)值。方法選擇經(jīng)B超檢查確診為單側(cè)腎積水的患者176例(輕度積水組21例、中度積水組33例、重度積水組122例),同期選取泌尿系統(tǒng)及GFR均正常的腎移植供體捐獻(xiàn)者22例(正常對(duì)照組)。兩組均接受99mTc-DTPA腎動(dòng)態(tài)顯像檢查,用Gates法計(jì)算患腎、健腎、雙腎GFR值,根據(jù)單腎GFR將患者腎功能劃分為腎功能正常、輕度下降、中度下降、重度下降及腎無功能。結(jié)果與正常對(duì)照組比較,中、重度積水組患側(cè)腎GFR降低,輕、中、重度積水組健側(cè)腎GFR升高,中、重度積水組雙腎GFR降低(P均0.05)。輕度積水組90.48%的患腎GFR正常,中度積水組63.64%的患腎GFR輕度下降,重度積水組35.25%的患腎GFR重度下降、33.33%的患腎無功能。積水越嚴(yán)重腎功能越差。結(jié)論 Gates法計(jì)算的單腎、總腎GFR可定量分析單側(cè)腎積水患者腎小球?yàn)V過功能,同步評(píng)價(jià)健側(cè)腎的代償能力。
[Abstract]:Objective to investigate the value of glomerular filtration rate (GFR) calculated by Gates method in evaluating glomerular filtration function in patients with unilateral hydronephrosis. Methods 176 patients with unilateral hydronephrosis (21 mild hydronephrosis and 33 moderate hydronephrosis) were selected. There were 122 patients with severe hydronephrosis and 22 donors with normal urinary system and GFR (normal control group). Both groups received 99mTc-DTPA renal dynamic imaging. The GFR values of affected kidney, healthy kidney and double kidney were calculated by Gates method. According to the single kidney GFR, the patients were divided into normal renal function, mild decline, moderate decline, severe decline and renal failure. Results compared with the normal control group, the patients with moderate and severe hydronephrosis had lower, mild and moderate renal GFR. In the severe hydronephrosis group, the GFR of the healthy kidney increased, while in the moderate and severe hydronephrosis group, the GFR of both kidneys decreased (P < 0.05). The GFR of the affected kidney in the mild hydronephrosis group (90.48%) was normal, while that in the moderate hydronephrosis group (63.64%) was slightly decreased. In severe hydronephrosis group (35.25%), GFR decreased significantly by 33.33%. The more severe hydronephrosis, the worse the renal function. Conclusion Gates method can quantitatively analyze glomerular filtration function in patients with unilateral hydronephrosis, and total renal GFR can be used to quantify glomerular filtration function in patients with unilateral hydronephrosis. The compensatory ability of healthy kidney was evaluated simultaneously.
