【摘要】：目的臂踝動脈脈搏波傳導速度(baPWV)是心血管病危險及預后的獨立預測因子之一,慢性腎臟病(CKD)患者心腦血管事件增加可能與其動脈硬化有關(guān),目前為止有關(guān)baPWV與CKD的研究結(jié)果尚不一致,本研究主要探討baPWV與CKD的關(guān)系。方法對完成baPWV檢測的觀察對象19 376人進行分析,平均年齡為50.5歲,其中男性12 533人(64.8%)。CKD包括估算的腎小球濾過率(eGFR)60 mL/(min·1.73 m~2)或尿蛋白陽性。結(jié)果 CKD的發(fā)生率隨baPWV(三分位)的升高而升高。在Logistic回歸分析中,校正了年齡、性別等混雜因素后,以baPWV第1三分位組為對照組,第3三分位組發(fā)生eGFR60mL/(min·1.73m~2)、尿蛋白陽性或CKD的OR值(95%CI)分別為1.34(1.05~1.70)、1.67(1.27~2.16)、1.46(1.21~1.77);以baPWV每增加一個標準差為自變量,校正了多種混雜因素后,發(fā)生eGFR60mL/(min·1.73m~2)、尿蛋白陽性、CKD的OR值(95%CI)分別為1.15(1.05~1.25)、1.24(1.14~1.35)、1.20(1.13~1.29)。結(jié)論 baPWV增加與CKD的發(fā)生獨立相關(guān)。
[Abstract]:Objective the brachial and malleolus pulse wave velocity (BAPWV) is one of the independent predictors of cardiovascular risk and prognosis. The increase of cardiovascular and cerebrovascular events in patients with chronic kidney disease may be related to arteriosclerosis. So far, the results about baPWV and CKD are not consistent. This study mainly discusses the relationship between baPWV and CKD. Methods 19 376 subjects who completed baPWV examination were analyzed, the average age was 50.5 years old. Among them, 12 533 men (64.8%) included estimated glomerular filtration rate (GFR) 60 mL/(min 路1.73 mm2) or urinary protein positive. Results the incidence of CKD increased with the increase of BaPWV3. In Logistic regression analysis, the age, sex and other mixed factors were adjusted. In the third position group, eGFR60mL/(min 路1.73 m-2, the OR value of urinary protein positive or CKD were 1.34 鹵1.051.70 and 1.67 1.272.16 ~ 1.46 ~ 1.21 ~ 1.77, respectively. After the addition of a standard deviation of baPWV as an independent variable, many mixed factors were corrected. The odds ratio of eGFR60mL/(min 路1.73 m ~ (2) and urinary protein positive was 1.15 ~ 1.05 ~ (1.25) ~ 1.24 ~ 1.14 ~ 1.35 ~ 1.20 ~ 1.131.29 ~ 0.Conclusion the increase of baPWV is independently related to the occurrence of CKD.
