低溫機(jī)械灌注與單純冷保存供腎對腎移植預(yù)后影響的Meta分析

【摘要】:背景:目前,國內(nèi)外低溫機(jī)械灌注與冷保存對腎移植預(yù)后影響的文章比較多,但結(jié)論各異,且由于大多單個(gè)研究樣本量存在局限,對低溫機(jī)械灌注的優(yōu)缺點(diǎn)缺乏客觀評價(jià)。目的:比較低溫機(jī)械灌注與冷保存對腎移植預(yù)后的影響。方法:計(jì)算機(jī)檢索Pub Med、sino Med、EMbase、Web of Science、the Cochrane Central Register of Controlled Trials(CENTRAL)、萬方數(shù)據(jù)庫及中國知網(wǎng),查找并篩選出所有比較低溫機(jī)械灌注與冷保存對腎移植預(yù)后影響的隨機(jī)對照試驗(yàn)(Randomized Controlled Trial,RCT),同時(shí)手檢納入文獻(xiàn)的參考文獻(xiàn),檢索時(shí)限均為建庫至2015年3月4日。按納入排除標(biāo)準(zhǔn)由2位評價(jià)者獨(dú)立進(jìn)行RCT的篩選、資料提取和質(zhì)量評價(jià)后,采用RevM an(5.3.0版)軟件進(jìn)行Meta分析,并采用GRADEpro3.6軟件進(jìn)行證據(jù)質(zhì)量評價(jià)。結(jié)果與結(jié)論:共納入文獻(xiàn)6篇,低溫機(jī)械灌注保存供腎共619例,冷保存保存供腎共620例。結(jié)果顯示:低溫機(jī)械灌注組原發(fā)性移植物無功能發(fā)生率、功能性移植腎功能延遲恢復(fù)發(fā)生率、移植腎功能延遲恢復(fù)發(fā)生率均顯著低于冷保存組(P0.05或0.01)。患者1年生存率、移植物1年存活率差異均無顯著性意義(P0.05)。結(jié)果表明低溫機(jī)械灌注保存供體只能降低腎移植后近期并發(fā)癥的發(fā)生率,維護(hù)腎臟功能,但無法提高患者1年生存率和移植物1年存活率,相比較冷保存的保存方式,低溫機(jī)械灌注對供體器官的保護(hù)無長期優(yōu)勢。
[Abstract]:Background: at present, domestic and international influence of low temperature mechanical perfusion and cold preservation on the prognosis of renal transplantation are many, but the conclusion is different, and because most of the single sample size limitations, lack of objective evaluation of the advantages and disadvantages of low temperature mechanical perfusion. Objective: To compare the effects of irrigation and low temperature mechanical injection of cold preservation on the prognosis of renal transplantation. Methods: We searched the Pub Med, Sino Med, EMbase Web, of Science, the Cochrane Central Register of Controlled Trials (CENTRAL), Wanfang database and Chinese HowNet, searching and screening all randomized controlled trials of relatively low mechanical perfusion and cold preservation on the prognosis of renal transplantation (Randomized Controlled Trial, RCT), at the same time hand inspection into literature references were searched from inception to March 4, 2015. Screening according to inclusion and exclusion criteria by 2 reviewers independently of RCT, data extraction and quality evaluation After using RevM an (5.3.0) Meta analysis software, and the evidence for the quality evaluation by GRADEpro3.6 software. Results and conclusion: a total of 6 articles, cold machine perfusion preservation of donor kidney in 619 cases, cold preservation of donor kidney in 620 cases. The results showed that low temperature mechanical perfusion group primary nonfunction the incidence of graft function, delayed graft function recovery rate, the incidence of delayed graft function were significantly lower than those in the cold preservation group (P0.05 or 0.01). The 1 year survival rate of patients and graft survival rate of 1 years there was no significant difference (P0.05). The results show that the low temperature mechanical perfusion preservation of donor only to reduce the complications after renal transplantation, the incidence of maintenance of renal function, but can not improve the 1 year survival rate of patients and graft survival rate of 1 years, compared with cold storage preservation, low temperature mechanical perfusion on donor organ protection without long-term advantage.
