【摘要】：目的評價輸尿管軟鏡(FURL)結合鈥激光碎石術同期治療雙側上尿路結石的安全性和有效性。方法回顧性分析我院2014年9月至2016年11月采用FURL同期治療雙側上尿路結石43例的臨床資料:所有患者術前行CT掃描,術中置入輸尿管軟鏡,使用200μm光纖,0.8~1.0J/10~20Hz功率進行碎石,配合使用取石網籃,碎石后常規(guī)留置雙側輸尿管支架管。術后1天復查KUB或CT,結石殘留者4-6周后取出輸尿管支架管時再次復查KUB。結果所有患者均一次性成功置入輸尿管軟鏡,平均手術時間101.5±37.2(25~180)min,結石清除率(SFR)總體為81.4%(35/43),結石負荷≥30mm組和30mm組分別為62.3%和100%(P=0.001)。術后1例出現(xiàn)發(fā)熱(39.5℃),1例出現(xiàn)腎包膜下血腫,無其他嚴重并發(fā)癥發(fā)生。術后住院時間平均為4天。結論FURL結合鈥激光碎石術是同期治療雙側上尿路結石的有效手段,尤其是對于結石負荷30mm者,具有較高清石率,安全性良好。
[Abstract]:Objective to evaluate the safety and efficacy of soft ureteroscopy (FURL) combined with holmium laser lithotripsy (Holmium laser lithotripsy) in the treatment of bilateral upper urinary calculi. Methods from September 2014 to November 2016, FURL was used to treat bilateral upper urinary calculi. Clinical data of 43 cases: all patients underwent CT scan before operation, A soft ureteroscope was inserted during the operation. The lithotripsy was performed with a 200 渭 m fiber optic fiber of 0.8 ~ 1.0 J / 10 ~ 20 Hz power, and a stone basket was used. After lithotripsy, bilateral ureteral stents were routinely placed. After 1 day, KUB or CTS were re-examined, and the ureteral stents were removed 4-6 weeks later.
