腎康注射液對老年慢性腎病患者造影劑腎病的防治作用

【摘要】:目的探討中藥制劑腎康注射液對老年慢性腎病患者造影劑腎病(contrast-induced nephropathy,CIN)防治的安全性和有效性。方法將206例老年慢性腎病擬行冠狀動脈造影的患者按照隨機數(shù)字表法分為水化組(67例)、腎康組(71例)和對照組(68例)。水化組手術(shù)前后12 h,均予0.9%氯化鈉注射液靜脈滴注。腎康組予腎康注射液靜脈滴注,對照組予靜脈滴注5%葡萄糖,兩組均術(shù)前3天及術(shù)后應(yīng)用4天,每天1次。觀察各組患者CIN的發(fā)生率,以及術(shù)前、術(shù)后24、96 h肌酐、尿素氮、血清胱抑素C(serum cystatin C,Cys C)、腎損傷分子-1(kidney injury molecule-1,KIM-1)和β2-微球蛋白(β2-microglobulin,β2-MG)腎功能指標(biāo)改變。通過Sperman相關(guān)性分析,比較年齡、性別、腎康注射液、造影劑用量、術(shù)前腎功能指標(biāo)水平與24 h肌酐變化值(術(shù)后24 h-術(shù)前)、CIN發(fā)生率的相關(guān)性,并進行Logistic回歸分析。結(jié)果與水化組及對照組比較,腎康組患者CIN發(fā)病率明顯降低,差異有統(tǒng)計學(xué)意義(χ2=5.32,P0.05)。與本組治療前比較,治療24、96 h后3組患者肌酐和Cys C水平均升高(P0.05),水化組和對照組KIM-1升高(P0.05),腎康組β2-MG水平升高(P0.05);與對照組比較,腎康組術(shù)后24 hβ2-MG升高(P0.05),其他指標(biāo)組間比較,差異均無統(tǒng)計學(xué)意義(P0.05)。腎康治療與CIN發(fā)生率和24 h肌酐變化呈負相關(guān)(r=-0.612、-0.517,P0.05)。造影劑用量與CIN發(fā)生率和24 h肌酐變化呈正相關(guān)(r=0.644、0.562,P0.05)。造影劑用量增加使CIN發(fā)生率升高(P0.05),腎康治療后CIN發(fā)生率明顯降低(P0.05)。結(jié)論中藥制劑腎康注射液對老年腎功能不全患者CIN起到一定的防治作用,降低CIN的發(fā)生率。
[Abstract]:Objective to investigate the traditional Chinese medicine preparation Shenkang injection of contrast agent nephropathy in elderly patients with chronic kidney disease (contrast-induced nephropathy, CIN) the safety and effectiveness of prevention and treatment.
