【摘要】：目的探討膀胱腫瘤抗原(BTA)的臨床應用價值。方法收集46例膀胱癌、35例泌尿系良性疾病、10例健康者尿液,同時做尿脫落細胞學及BTA檢測。結果 (1)BTA檢測的敏感性為86.9%,特異性為82.2%,假陽性率為17.8%(95%CI:0.759,0.924);尿脫落細胞學檢測的敏感性為34.7%,特異性為97.8%,假陽性率為2.2%(95%CI:0.550,0.775);(2)BTA表達水平和敏感性與腫瘤分級分期、有無肉眼血尿有關(P0.05),與復發(fā)或初發(fā)無關(P0.05);BTA對各級、各期的膀胱癌的敏感性均高于尿脫落細胞學檢測(P0.05);(3)BTA在早期膀胱癌(Ta~T1期)中表達水平高于良性疾病組、健康組(P0.05)。結論 BTA對膀胱癌的診斷、早期篩查及跟蹤隨訪有較高的臨床應用價值,但應注意血尿影響B(tài)TA檢測結果。
[Abstract]:Objective to evaluate the clinical value of bladder tumor antigen (BTAA). Methods urine samples from 35 patients with bladder cancer and 10 healthy controls were collected. Urine exfoliative cytology and BTA were performed simultaneously. Results the sensitivity and specificity of BTA were 86.9 and 82.2% respectively. The false positive rate was 17. 8% and 95%, including 0. 759% and 0. 924%. The sensitivity, specificity and false positive rate of urine exfoliative cytology were 34.7 and 97.8.The false positive rate was 2.2and 95 CI: 0.550 / 0.775; The expression level and sensitivity of BTA were correlated with tumor grade and stage, with or without gross hematuria (P0.05A), and had no relation with recurrence or initial onset (P0.05A). The sensitivity of BTA to bladder cancer at all stages was higher than that of urine exfoliative cytology (P0.05). The expression of BTA in the early stage of bladder cancer was higher than that in the benign group. Conclusion BTA is useful in the diagnosis of bladder cancer. Early screening and follow-up have high clinical value, but we should pay attention to the influence of hematuria on the results of BTA.
