【摘要】：目的探討血液稀釋聯(lián)合控制性降壓對(duì)膝關(guān)節(jié)置換手術(shù)病人認(rèn)知功能的影響。方法選擇膝關(guān)節(jié)置換手術(shù)病人50例,隨機(jī)分為對(duì)照組(Ⅰ組)和觀察組(Ⅱ)兩組,每組25例。Ⅰ組:氣囊止血帶控制下進(jìn)行手術(shù)。Ⅱ組:血液稀釋聯(lián)合控制性降壓下行手術(shù),調(diào)節(jié)平均動(dòng)脈壓于60~75 mmHg之間。分別于術(shù)前1 d和術(shù)后3 d利用簡(jiǎn)易智能量表對(duì)患者進(jìn)行神經(jīng)功能評(píng)分(MMSE),分別于麻醉誘導(dǎo)前(T_0)、術(shù)畢(T_1)、術(shù)后6-8 h(T_2)3個(gè)時(shí)間點(diǎn)抽取靜脈血1 mL,測(cè)量血清中slOOβ蛋白的含量,評(píng)價(jià)術(shù)后認(rèn)知功能的發(fā)生。結(jié)果兩組術(shù)后MMSE評(píng)分及血清slOOβ蛋白含量無(wú)明顯差異。結(jié)論血液稀釋聯(lián)合控制性降壓不增加膝關(guān)節(jié)置換患者術(shù)后認(rèn)知功能障礙的發(fā)生。
