星狀神經(jīng)節(jié)阻滯對(duì)SD大鼠離體心功能的影響

【摘要】:目的:SD大鼠行星狀神經(jīng)節(jié)阻滯,觀察星狀神經(jīng)節(jié)阻滯對(duì)Langendorff離體心臟灌流裝置離體心功能的影響,為星狀神經(jīng)節(jié)阻滯用于臨床改善心功能減少圍手術(shù)期心血管事件發(fā)生提供理論依據(jù)。 方法:將24只8~10周齡體重180~220克的雄性SD大鼠采用隨機(jī)數(shù)字表法隨機(jī)分為3組,每組8只:(1)左側(cè)星狀神經(jīng)節(jié)阻滯組(LS組);(2)右側(cè)星狀神經(jīng)節(jié)阻滯組(RS組);(3)手術(shù)對(duì)照組(C組)。大鼠腹腔內(nèi)注射完成麻醉后,鈍性分開左右胸頭肌,沿食管旁找到星狀神經(jīng)節(jié),埋管并縫皮,7天后行星狀神經(jīng)節(jié)阻滯實(shí)驗(yàn),觀察是否出現(xiàn)霍納綜合征,并連續(xù)給藥兩周。星狀神經(jīng)節(jié)阻滯模型建立成功后,應(yīng)用Langendorff離體心臟灌流裝置建立SD大鼠離體心臟灌流模型。大鼠腹腔注射肝素后,仰臥位固定,迅速開胸,取出心臟,置于4℃冷的K-H液中分離主動(dòng)脈,懸掛于Langendorff離體心臟灌流裝置上。K-H液灌注15~20分鐘,達(dá)到平衡末時(shí)測(cè)定冠脈流量(CF),記錄相關(guān)的心功能指標(biāo):心率(HR)左室發(fā)展壓(LVDP)左室舒張末壓(LVEDP)收縮期心室內(nèi)壓力升高速率(+dp/dt)舒張期心室內(nèi)壓力升高速率(-dp/dt)。同時(shí)取下心臟離心后取上清,經(jīng)過加標(biāo)準(zhǔn)品加樣加抗體洗滌以及加底物等步驟后,用ELISA法檢測(cè)心肌組織中腎上腺素(EPI)去甲腎上腺素(NE)神經(jīng)肽Y(NPY)和血管活性腸肽(VIP)的濃度。 結(jié)果:1.與C組相比,RSGB組LVDP+dp/dtmax-dp/dtmaxCF及HR值均升高有統(tǒng)計(jì)學(xué)意義(P<0.05或0.01),RSGB組的LVEDP值降低有統(tǒng)計(jì)學(xué)意義(P<0.01);與C組相比,LSGB組LVDP+dp/dtmax-dp/dtmax及CF值均升高有統(tǒng)計(jì)學(xué)意義(P<0.05或0.01),LSGB組的LVEDP值降低有統(tǒng)計(jì)學(xué)意義(P<0.05)。與LSGB組相比,RSGB組LVDP+dp/dtmax及-dp/dtmax值升高有統(tǒng)計(jì)學(xué)意義(P<0.05),RSGB組LVEDP值降低有統(tǒng)計(jì)學(xué)意義(P<0.05)。2.與C組相比,RSGB組和LSGB組EPINENPY及VIP值均升高有統(tǒng)計(jì)學(xué)意義(P<0.05或0.01)。與LSGB組相比,RSGB組EPINE及NPY值均升高有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論:1.星狀神經(jīng)節(jié)阻滯通過增強(qiáng)心肌的收縮和舒張功能,對(duì)SD大鼠離體心功能有促進(jìn)作用;2.左右兩側(cè)星狀神經(jīng)節(jié)對(duì)心臟的交感神經(jīng)支配區(qū)域不同,,兩側(cè)星狀神經(jīng)節(jié)阻滯對(duì)大鼠離體心功能的影響存在差異,右側(cè)星狀神經(jīng)節(jié)阻滯增強(qiáng)心功能的效果更佳。
