【摘要】：目的采用術(shù)后恢復(fù)質(zhì)量量表(PQRS)分析全身麻醉術(shù)后患者麻醉滿意度的影響因素。方法選擇擇期在全身麻醉下行非心腦手術(shù)的成年患者369例,于術(shù)后24、72和168h對患者的麻醉滿意度進行調(diào)查并進行PQRS評估�；颊邼M意度分為完全滿意、一般、不太滿意和非常不滿意,任意時間點均完全滿意者納入滿意組,其余患者納入不滿意組。對與患者不滿意相關(guān)的指標進行單因素分析和Logistic多因素非條件回歸分析。結(jié)果 369例患者中,332例(90.0%)納入滿意組,37例(10.0%)納入不滿意組。滿意組術(shù)前焦慮發(fā)生率和麻醉時間顯著低于不滿意組(P值均0.01)。滿意組的蘇醒期躁動(Riker鎮(zhèn)靜/躁動評分5~7分)發(fā)生率、術(shù)后24h惡心嘔吐(Likert評分≥3分)發(fā)生率、術(shù)后24和72h疼痛(Likert評分≥3分)發(fā)生率、術(shù)后168h認知恢復(fù)率(整體評分基礎(chǔ)值80%)均顯著低于不滿意組(P值均0.01)�；颊呗樽頋M意度不高的原因從主到次依次為術(shù)后24h惡心嘔吐(OR=4.82,95%CI為1.85~12.59,P=0.001)、術(shù)前焦慮(OR=3.79,95%CI為1.60~8.94,P=0.002)、術(shù)后72h疼痛(OR=3.59,95%CI為1.30~9.93,P=0.014)、術(shù)后24h疼痛(OR=3.38,95%CI為1.42~8.03,P=0.006)、術(shù)后168h認知恢復(fù)不良(OR=2.84,95%CI為1.31~6.18,P=0.008)和麻醉持續(xù)時間長(OR=1.79,95%CI為1.06~3.04,P=0.030)。結(jié)論 PQRS分析提示,導(dǎo)致全身麻醉患者不滿意的最主要原因是術(shù)后惡心嘔吐,其次是術(shù)后疼痛、術(shù)前焦慮、麻醉時間長和術(shù)后1周內(nèi)認知仍不能完全恢復(fù)。
[Abstract]:Objective to analyze the influencing factors of postoperative anesthetic satisfaction by using postoperative recovery quality scale (PQRS). Methods A total of 369 adult patients undergoing non-cardiac and cerebral surgery under general anesthesia were investigated and evaluated with PQRS at 2472 and 168h after operation. The patients' satisfaction was divided into complete satisfaction, general, unsatisfactory and very unsatisfactory. At any time, those who were completely satisfied were included in the satisfaction group, while the other patients were included in the unsatisfactory group. Univariate analysis and Logistic multivariate non-conditional regression analysis were performed for the indicators related to patients' dissatisfaction. Results 332 cases (90.0%) were included in satisfactory group and 37 cases (10.0%) in unsatisfactory group. The incidence of preoperative anxiety and anaesthesia time in the satisfactory group were significantly lower than those in the unsatisfactory group (P < 0.01).
