【摘要】：目的:探究分析經(jīng)皮穴位電刺激復(fù)合腰硬聯(lián)合麻醉在下肢骨科手術(shù)中的鎮(zhèn)靜鎮(zhèn)痛效果。方法:選擇我院2013年3月—2014年5月收治的下肢骨科手術(shù)患者72例,在患者知情同意的情況下將其隨機(jī)分為兩組:即經(jīng)皮穴位電刺激組與對(duì)照組各36例。對(duì)比分析兩組患者腰硬聯(lián)合麻醉后的臨床效果。結(jié)果:經(jīng)皮穴位電刺激組患者手術(shù)時(shí)間為(74.36±21.4)min,麻醉平面為(6.0±1.2)T,術(shù)中失血量為(118.6±2.7)m L,對(duì)照組患者手術(shù)時(shí)間為(76.12±19.5)min,麻醉平面為(6.0±0.9)T,術(shù)中失血量為(119.1±2.2)m L。兩組患者麻醉平面、手術(shù)時(shí)間以及術(shù)中失血量情況對(duì)比差異不顯著,無(wú)統(tǒng)計(jì)學(xué)意義(P=0.34,t=4.74)。兩組患者在麻醉前以及手術(shù)開始時(shí)血壓、心率、Sp O2、BIS等情況對(duì)比無(wú)顯著差異(P=0.79,t=6.21);而在手術(shù)開始后半小時(shí)以及手術(shù)后,經(jīng)皮穴位電刺激組患者的BIS較對(duì)照組明顯降低(P=0.03,t=1.14)。經(jīng)皮穴位電刺激組術(shù)后8 h以及術(shù)后24 h的VAS評(píng)分,與對(duì)照組相比明顯更低,兩組對(duì)比差異有統(tǒng)計(jì)學(xué)意義(P=1.26,t=6.03)。兩組不良反應(yīng)發(fā)生情況對(duì)比差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.35,t=5.27)。結(jié)論:經(jīng)皮穴位電刺激復(fù)合腰硬聯(lián)合麻醉在下肢骨科手術(shù)中能夠起到較好的鎮(zhèn)靜鎮(zhèn)痛效果,減少了鎮(zhèn)痛藥物的用量,且不良反應(yīng)發(fā)生率較低。
[Abstract]:Objective: To explore the sedative and analgesic effects of the combined lumbar and epidural anesthesia in the operation of the lower limb. Methods: 72 patients with lower limb and bone surgery from March 2013 to May 2014 were selected, and were randomly divided into two groups with the consent of the patient: the electrostimulation group and the control group were divided into two groups. To compare the clinical effects of the two groups in the treatment of lumbar and hard combined anesthesia. Results: The operative time of the electrostimulation group was (74.36-22.4) min, the level of anesthesia was (6.0-1.2) T, the amount of blood loss during the operation was (118. 6-2.7) m-L, the operation time of the control group was (72.12-195min) min, and the anesthetic plane was (6.0-0.9) T. The amount of blood loss during the operation was (119. 1, 2. 2) m L. There was no significant difference in the level of anesthesia, the time of operation and the amount of intraoperative blood loss in the two groups (P = 0.34, t = 4.74). There was no significant difference in blood pressure, heart rate, Sp 2, BIS and other cases before and after anesthesia (P = 0.79, t = 6.21). The VAS scores at 8 h after operation and 24 hours after operation were significantly lower than that in the control group (P = 1.26, t = 6.03). There was no significant difference between the two groups (P = 0.35, t = 5.27). Conclusion: The combined lumbar and epidural anesthesia can play a better sedative and analgesic effect in the operation of the lower limb, and the dosage of the analgesic drug is reduced, and the incidence of adverse reactions is low.
