【摘要】：目的觀察靜脈輸注利多卡因?qū)Ω骨荤R全子宮切除術(shù)后疼痛及早期康復(fù)的影響。方法96例擇期行腹腔鏡全子宮切除術(shù)的患者,隨機(jī)分為研究組和對照組,每組48例。研究組麻醉誘導(dǎo)時(shí)靜脈給予2%利多卡因1.5mg/kg,隨后連續(xù)靜脈輸注利多卡因2mg·kg~(-1)·h(-1)直至手術(shù)結(jié)束,對照組以生理鹽水代替利多卡因。記錄術(shù)后6、12、24和48h靜息與咳嗽狀態(tài)下VAS評分,48h內(nèi)嗎啡累積使用量,術(shù)后第一次排氣時(shí)間,術(shù)后第3~5天每天六分鐘步行測試距離,以及術(shù)后惡心、嘔吐、眩暈等不良反應(yīng)情況。結(jié)果術(shù)后6、12和24h靜息狀態(tài)下研究組VAS評分明顯低于對照組(P0.05);術(shù)后48h內(nèi)各時(shí)點(diǎn)咳嗽狀態(tài)下研究組VAS評分明顯低于對照組(P0.05)。與對照組比較,研究組48h內(nèi)嗎啡累積使用量明顯減少(P0.05),術(shù)后第一次排氣時(shí)間明顯縮短(P0.05),步行測試距離明顯延長(P0.05)。兩組術(shù)后不良反應(yīng)差異無統(tǒng)計(jì)學(xué)意義。結(jié)論靜脈輸注利多卡因可以減輕腹腔鏡全子宮切除術(shù)后疼痛,減少嗎啡用量,促進(jìn)患者早期康復(fù)。
[Abstract]:Objective to observe the effect of intravenous lidocaine on pain and early recovery after laparoscopic hysterectomy. Methods 96 patients undergoing laparoscopic hysterectomy were randomly divided into study group and control group with 48 cases in each group. In the study group, 2% lidocaine was given intravenously at 1.5 mg / kg during anesthesia induction, and then lidocaine (-1) h (-1) was continuously infused intravenously until the end of the operation. In the control group, normal saline was used instead of lidocaine. The VAS scores of rest and cough at 24 and 48 hours after operation, the cumulative use of morphine within 48 hours, the first time of exhaust after operation, the walking distance of 6 minutes per day on the 3rd day of operation, and the adverse reactions such as nausea, vomiting and vertigo were recorded. Results the VAS score of the study group was significantly lower than that of the control group (P0.05), and the VAS score of the study group was significantly lower than that of the control group (P0.05) under the cough state at every time point within 48 hours after operation (P0.05). Compared with the control group, the cumulative use of morphine decreased significantly in the study group within 48 hours (P0.05), the first time of exhaust after operation was significantly shortened (P0.05), and the distance of walking test was significantly prolonged (P0.05). There was no significant difference in postoperative adverse reactions between the two groups. Conclusion intravenous infusion of lidocaine can relieve the pain after laparoscopic hysterectomy, reduce the dosage of morphine, and promote the early recovery of patients.
