【摘要】：正建立中心靜脈通路在臨床上具有重要意義,臨床多選用右側(cè)頸內(nèi)靜脈穿刺。傳統(tǒng)的頸內(nèi)靜脈穿刺插管徑路很多,但并發(fā)癥也較多,如誤傷動(dòng)脈、神經(jīng)及氣胸、血胸、血腫和導(dǎo)管位置異常等發(fā)生率可高達(dá)10%,頸內(nèi)靜脈置管失敗率可超過19%[1]。因此,提高頸內(nèi)靜脈穿刺成功率,減少其并發(fā)癥的發(fā)生率非常重要。研究表明[2],在麻醉后使用呼氣末正壓可以提高頸內(nèi)靜脈充盈度,從而提高穿刺成功率;但對(duì)于清醒患者能否通過Valsalva動(dòng)作提高患者頸內(nèi)靜脈的充盈度,進(jìn)而有利于穿刺成功率,尚未見文獻(xiàn)報(bào)
[Abstract]:The establishment of central venous access is of great clinical significance, clinical use of the right internal jugular vein puncture. There are many traditional internal jugular vein catheterization paths, but there are also many complications, such as accidental injury of artery, nerve and pneumothorax, hematoma and abnormal location of catheter, etc. The failure rate of internal jugular vein catheterization can exceed 19% [1]. Therefore, it is important to improve the success rate of internal jugular vein puncture and reduce the incidence of complications. The results show that [2] after anesthesia, positive end-expiratory pressure can improve the filling degree of internal jugular vein, thus increasing the success rate of puncture. However, whether the awake patients can improve the filling degree of internal jugular vein through Valsalva, which is beneficial to the success rate of puncture, has not been reported in the literature.
