【摘要】：目的探討不同麻醉趨勢(Narcotrend)分級(jí)指導(dǎo)下的全身麻醉對老年腹部手術(shù)患者術(shù)后認(rèn)知功能的影響。方法擇期全麻下行腹部手術(shù)老年患者150例,隨機(jī)均分為三組(A、B和C組)。以咪達(dá)唑侖0.05 mg/kg、丙泊酚1 mg/kg、瑞芬太尼2μg/kg及羅庫溴銨0.6 mg/kg誘導(dǎo)氣管插管,術(shù)中以瑞芬太尼0.15μg/(kg·min)、順式阿曲庫銨及丙泊酚維持麻醉。調(diào)節(jié)丙泊酚泵注速度使A、B、C三組麻醉深度維持至相應(yīng)水平(D064~57、D156~47、D246~37)。記錄手術(shù)期間的Narcotrend分級(jí)(NT)及指數(shù)(NI),采用BIS作為平行對照。同時(shí)記錄患者蘇醒睜眼時(shí)間、拔管時(shí)間及各個(gè)時(shí)間點(diǎn)的心率和血壓變化。分別于術(shù)前1 d和術(shù)后第7天使用簡易精神狀態(tài)量表(MMSE)及蒙特利爾認(rèn)知功能量表(MoCA)對患者進(jìn)行認(rèn)知功能評(píng)估。結(jié)果①A、B和C組患者術(shù)后認(rèn)知功能障礙(POCD)發(fā)生率分別為20.8%、15.2%和13.3%,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。②A、B和C組患者術(shù)前及術(shù)中各個(gè)時(shí)間點(diǎn)的心率和血壓比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。③三組患者拔管時(shí)間有顯著差異(P0.05),B比A、C組蘇醒拔管快。結(jié)論不同Narcotrend分級(jí)指導(dǎo)下全身麻醉對老年患者腹部手術(shù)術(shù)后認(rèn)知功能無顯著影響,處于D1級(jí)麻醉深度時(shí)蘇醒拔管早。
[Abstract]:Objective to investigate the effect of general anesthesia with different anesthetic trends under (Narcotrend) classification on postoperative cognitive function in elderly patients undergoing abdominal surgery. Methods 150 elderly patients undergoing abdominal surgery under general anesthesia were randomly divided into three groups: group A (group B) and group C (group C). Midazolam 0.05 mg/kg, propofol 1 mg/kg, remifentanil 2 渭 g/kg and rocuronium 0.6 mg/kg were used to induce tracheal intubation. Anesthesia was maintained with remifentanil 0.15 渭 g / (kg min), cis atracurium and propofol. Adjusting the injection speed of propofol pump to maintain the anesthesia depth to the corresponding level (D06457-D156N 47 D246C37). Narcotrend grading (NT) and index (NI), were recorded during operation using BIS as parallel control. At the same time, the changes of heart rate and blood pressure at each time point were recorded. (MMSE) and (MoCA) were used to evaluate the cognitive function of the patients 1 day before operation and 7 days after operation. Results 1the incidence of (POCD) in patients with cognitive dysfunction in group A, B and C was 20. 8% and 13. 3%, respectively. There was no significant difference in heart rate and blood pressure between group B and group C (P0.05). There was no significant difference in heart rate and blood pressure between group C and group B (P0.05). 3There was significant difference in extubation time among the three groups (P0.05). Conclusion General anesthesia under the guidance of different Narcotrend grades has no significant effect on cognitive function after abdominal surgery in elderly patients.
